静脉全身麻醉联合超声引导下阴茎背神经阻滞在小儿包皮环切手术中的应用分析

Application of intravenous general anesthesia combined with ultrasound-guided dorsal penile nerve block to pediatric circumcision

目的:分析静脉全身麻醉联合超声引导下阴茎背神经阻滞在小儿包皮环切手术中的效果。方法:选取100例行包皮环切手术的患儿作为研究对象,根据麻醉方式不同将患儿分成实验组与对照组,每组各50例。对照组应用静脉全身麻醉联合传统阴茎背神经阻滞,实验组则应用静脉全身麻醉联合超声引导下阴茎背神经阻滞。比较两组患儿麻醉效果。结果:实验组患儿在切皮时(T_(1))、缝合即刻(T_(2))及术毕(T_(3))的HR、MAP水平低于对照组(P<0.05);实验组患儿手术时间、术后苏醒时间与术后留置恢复室时间均短于对照组(P<0.05),实验组患儿芬太尼用量少于对照组,两组比较差异有统计学意义(P<0.05);实验组不良反应总发生率明显低于对照组,两组比较差异有统计学意义(P<0.05)。结论:针对接受小儿包皮环切手术治疗的患儿,采取静脉全身麻醉联合超声引导下阴茎背神经阻滞,可以有效降低患儿手术过程的生命体征波动,减少麻醉药用量及缩短手术时间,术后恢复快、不良反应发生率低。

Objective:To investigate the effect of intravenous general anesthesia combined with ultrasound-guided penile dorsal nerve block on children undergoing circumcision.Methods:A total of 100 children undergoing circumcision were selected as the research objects,and divided into the experimental group and control group according to different anesthesia methods,each with 50 cases.The control group were treated with intravenous general anesthesia combined with traditional penile dorsal nerve block,while the experimental group were treated with intravenous general anesthesia combined with ultrasound-guided penile dorsal nerve block.The anesthetic effect was compared between the two groups.Results:HR and MAP levels in the experimental group were significantly lower than those in the control group at the time of skin resection(T_(1)),immediately after suturing(T_(2))and after operation(T_(3))(P<0.05);the operation time,postoperative recovery time and indwelling recovery room time of patients in the experimental group were significantly shorter than that of the control group,and the amount of fentanyl used in the experimental group was significantly less than the control group,which were of statistical differences(P<0.05);and the incidence of adverse reactions in the experimental group was significantly lower than that in the control group,which was of statistical difference(P<0.05).Conclusion:For children undergoing pediatric circumcision,intravenous general anesthesia combined with ultrasound-guided penile dorsal nerve block can effectively reduce the fluctuation of vital signs during the operation,reduce the dosage of anesthetics and shorten the operation time.At the same time,it has the advantage of quicker postoperative recovery and lower incidence of adverse reactions.
