药师干预对老年高血压出院患者用药依从性与疗效的影响

Effects of Pharmacist Intervention on the Efficacy and Medication Compliance of Elderly Discharged Patients with Hypertension

目的探讨药师干预对老年高血压出院患者用药依从性及疗效的影响。方法共纳入我院心血管内科收治的105例老年高血压患者作为研究对象,随机分为对照组和干预组。干预组由临床药师定期开展高血压知识宣教、用药教育、生活方式指导等药学服务;对照组仅进行定期随访,获取患者相关的研究数据。采用Morisky依从性量表(MMAS-4)评估比较两组患者的用药依从性,并对比两组患者的血压控制情况及不良反应情况等指标。结果出院后随访3个月时,干预组患者的用药依从性评分明显优于对照组(t=-3.614,P<0.05),血压控制达标率为94.44%,明显优于对照组(χ^(2)=5.987,P<0.05)。两组患者的药物不良反应发生情况无明显差异。结论药师干预可提高老年高血压患者的用药依从性,改善血压控制情况,值得借鉴推广。

OBJECTIVE To explore the effect of pharmacist intervention on medication compliance and curative effect of elderly discharged patients with hypertension.METHODS 105 elderly patients with hypertension treated in the department of cardiovascular medicine of our hospital were randomly divided into control group and intervention group.In the intervention group,clinical pharmacists regularly carried out pharmaceutical care such as hypertension knowledge education,medication education and lifestyle guidance and so on.While the control group was only followed up regularly to obtain relevant research data.Morisky medication adherence scale-4 items(MMAS-4)was used to evaluate the medication compliance of patients in the two groups,and the blood pressure control and adverse reactions were compared.RESULTS At the 3 rd month of follow-up after discharge,the medication compliance of patients in the intervention group was significantly better than the control group(t=-3.614,P<0.05),and the standard rate of blood pressure control was 94.44%,which was significantly better than the control group(χ^(2)=5.987,P<0.05).There was no significant difference in the incidence of adverse drug reactions between the two groups.CONCLUSION Pharmacist intervention can improve medication compliance and blood pressure control in elderly patients with hypertension,which is worthy of reference and promotion.
