重复经颅磁刺激联合盐酸度洛西汀肠溶片治疗恶性肿瘤合并抑郁症的效果评价

Effect evaluation of repetitive transcranial magnetic stimulation combined with duloxetine hydrochloride enteric-coated tablets in the treatment of malignant tumors complicated with depression

目的评价重复经颅磁刺激(rTMS)联合盐酸度洛西汀肠溶片治疗恶性肿瘤合并抑郁症患者的临床效果。方法80例恶性肿瘤合并抑郁症患者,随机分为对照组和观察组,每组40例。对照组使用盐酸度洛西汀肠溶片治疗,观察组在对照组基础上加用rTMS治疗。比较两组患者治疗效果及治疗前后汉密顿抑郁量表(HAMD)及抑郁症自我评估量表(PHQ-9)评分。结果观察组治疗总有效率为87.5%,高于对照组的60.0%,差异有统计学意义(P<0.05)。治疗前,两组HAMD评分及PHQ-9评分比较,差异无统计学意义(P>0.05);治疗8周后,两组HAMD评分及PHQ-9评分均低于治疗前,且观察组HAMD评分(8.16±1.62)分及PHQ-9评分(7.34±3.12)分低于对照组的(15.75±2.66)、(14.27±5.16)分,差异均有统计学意义(P<0.05)。结论rTMS联合盐酸度洛西汀肠溶片治疗恶性肿瘤合并抑郁症患者的疗效显著,可有效改善患者的抑郁焦虑症状,值得临床推广。

Objective To evaluate the clinical effect of repetitive transcranial magnetic stimulation(rTMS)combined with duloxetine hydrochloride enteric-coated tablets in the treatment of malignant tumors complicated with depression.Methods A total of 80 patients with malignant tumors and depression were randomly divided into control group and observation group,with 40 cases in each group.The control group was treated with duloxetine hydrochloride enteric-coated tablets,and the observation group was additionally treated with rTMS on the basis of the control group.The therapeutic effect,Hamilton Depression Scale(HAMD)and Patient Health Questionnaire-9(PHQ-9)scores before and after treatment were compared between the two groups.Results The total effective rate in the observation group was 87.5%,which was higher than 60.0% in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference in HAMD score and PHQ-9 score between the two groups(P>0.05).After 8 weeks of treatment,the HAMD scores and PHQ-9 scores of the two groups were lower than those of this group before treatment,and the HAMD score(8.16±1.62)points and PHQ-9 score(7.34±3.12)points of the observation group were lower than(15.75±2.66)and(14.27±5.16)points of the control group,and the difference was statistically significant(P<0.05).Conclusion r TMS combined with duloxetine hydrochloride enteric-coated tablets is effective in the treatment of patients with malignant tumor and depression,and it can effectively improve the symptoms of depression and anxiety in patients,which is worthy of clinical promotion.
