CT与MRI在老年脑出血患者中的诊断价值分析 被引量:2

摘要 目的探究CT与磁共振成像(MRI)在老年脑出血患者中的诊断价值。方法回顾性选择208例老年脑出血患者进行分析,患者均进行CT与MRI检查,比较两种检查方法的病灶部位诊断符合情况、诊断总符合率、漏诊率、误诊率。结果MRI和CT对基底节出血的诊断符合率比较差异无统计学意义(P>0.05);MRI对壳核出血、脑叶出血、丘脑出血的诊断符合率分别为85.11%、96.23%、89.80%,均明显高于CT的65.96%、77.36%、57.14%,差异具有统计学意义(P<0.05)。MRI的诊断总符合率90.87%高于CT的72.60%,漏诊率9.13%和误诊率0均低于CT的25.48%、1.92%,差异具有统计学意义(P<0.05)。结论MRI检查对于老年脑出血诊断准确度较CT更高,但临床应严格把握不同检查方式的影像学特征,减少误诊、漏诊情况。 Objective To investigate the value of CT and magnetic resonance imaging(MRI)in the diagnosis of elderly patients with intracerebral hemorrhage.Methods A total of 208 elderly patients with cerebral hemorrhage were retrospectively selected for analysis,and all patients underwent CT and MRI examinations.The diagnostic compliance of lesion sites,the total diagnostic compliance rate,the missed diagnosis rate,and the misdiagnosis rate were compared between the two examination methods.Results There was no statistically significant difference in the diagnostic coincidence rate between MRI and CT for basal ganglia hemorrhage(P>0.05).The diagnostic coincidence rates of MRI for putamen hemorrhage,lobar hemorrhage,and thalamic hemorrhage were 85.11%,96.23%,and 89.80%,which were significantly higher than 65.96%,77.36%,and 57.14% of CT,and the differences were statistically significant(P<0.05).The overall diagnostic coincidence rate 90.87% of MRI was higher than 72.60% of CT,and the missed diagnosis rate 9.13%and the misdiagnosis rate 0 were lower than 25.48% and 1.92% of CT,and the differences were statistically significant(P<0.05).Conclusion MRI examination is more accute than CT for the diagnosis of cerebral hemorrhage in the elderly,but the clinical imaging characteristics of different examination modalities should be strictly grasped to reduce misdiagnosis and missed diagnosis.
