心理干预联合小剂量阿立哌唑控制抽动障碍的疗效分析

Analysis of Curative Effect of Psychological Intervention Combined with Low-dose Aripiprazole in Controlling Tic Disorder

目的:观察心理干预联合小剂量阿立哌唑治疗儿童抽动障碍的疗效。方法:选取佳木斯市中心医院癫痫科2011年11月—2017年3月收活的46例抽动障碍(tic disorders,TD)患儿作为研究对象,按照随机分组原则分为小剂量阿立哌唑(aripiprazole,APZ)+心理干预组(23例)及小剂量APZ组(23例),治疗12周。采用耶鲁综合抽动严重程度量表(The Yale Global Tic Severity Scale Modified,YGTSS)于入组时、治疗12周时对患者进行评估。24周时随访复发率。结果:治疗12周,两组患儿运动抽动及发声抽动均明显减少,但小剂量APZ+心理干预组YGTSS减分率明显优于APZ组,差异有统计学意义(P<0.05)。24周时对患者进行随访发现APZ组复发率明显高于小剂量APZ+心理干预组,差异有统计学意义(P<0.05)。结论:心理干预在抽动障碍的治疗中具有重要地位,必须加以重视。

Objective:To observe the effect of psychological intervention combined with low-dose aripiprazole in the treatment of tic disorder in children.Methods:46 children with tic disorder(TD)who were treated in the hospital from November 2011 to March 2017 were selected and randomly divided into low-dose aripiprazole(APZ)+psychological intervention group(23 cases)and low-dose APZ group(23 cases)according to the principle of random.The treatment lasted for 12 weeks.The Yale Global Tic Severity Scale modified(YGTSS)was used to evaluate the patients at the time of enrollment and 12 weeks of treatment.The recurrence rate was followed up at 24 weeks.Results:At the end of 12 weeks of treatment,the motor tic and vocal twitch of the two groups significantly reduced,but the reduction rate of YGTSS in the low-dose APZ+psychological intervention group was significantly better than that in the APZ group.The patients were followed up at 24 weeks and found that the recurrence rate of APZ group was significantly higher than that of low-dose APZ+psychological intervention group,the difference was statistically signifieant(P<0.05).Conclusion:Psychological intervention plays an important role in the treatment of tic disorder,which should be paid attention to.
