PDCA循环管理对手指皮肤缺损皮瓣修复患者手部功能恢复及并发症的影响研究

Effect of PDCA Circulation Management on Hand Function Recovery and Complications of Patients with Finger Skin Defect and Flap Repair

目的:探讨PDCA循环管理对手指皮肤缺损皮瓣修复患者手部功能恢复及并发症的影响。方法:选择2018年5月―2020年5月佛山市南海区公共卫生医院收治的88例手指皮肤缺损皮瓣修复患者作为研究对象,采用简单随机法分为对照组和观察组,每组各44例。对照组实施常规护理,观察组实施PDCA循环管理模式,比较两组患者干预前后手指关节活动度、日常活动能力、并发症发生情况及患者满意度。结果:干预后,观察组手指关节被动度为(88.76±5.98)°、主动活动度为(90.32±7.25)°,高于对照组的(79.28±6.45)°、(77.41±8.54)°,差异有统计学意义(t=7.149、7.644,P<0.05)。干预后,观察组吃饭评分为(12.83±0.49)分、洗澡评分为(4.46±0.92)分、穿衣评分为(12.59±0.63)分、修饰评分为(11.89±0.74)分,高于对照组的(10.68±1.54)分、(11.09±1.34)分、(10.13±1.87)分、(10.03±1.02)分,差异有统计学意义(t=8.825、8.685、8.269、9.791,P<0.05)。观察组并发症发生率为6.82%,低于对照组的22.73%,差异有统计学意义(χ^(2)=4.423,P<0.05)。观察组患者总满意度为95.45%,高于对照组的77.27%,差异有统计学意义(χ^(2)=6.175,P<0.05)。结论:PDCA循环管理在改善手指皮肤缺损皮瓣修复患者手部功能恢复方面效果显著,有利于降低术后并发症发生情况,提高患者满意度。

Objective:To investigate the effect of PDCA circulation management on the recovery of hand function and complications in patients with finger skin defect flap repair.Methods:From May 2018 to May 2020,88 patients with finger skin defect and flap repair in the hospital were selected and divided into two groups according to the random number table method,with 44 cases in each group.The control group received routine nursing care,and the observation group adopted the PDCA circulation management model.The range of motion of finger joints,daily activities,complications and patient satisfaction were compared between the two groups before and after intervention.Results:After the intervention,the passive degree of finger joints in the observation group was(88.76±5.98)°,and the active range of motion was(90.32±7.25)°,which were higher than(79.28±6.45)°and(77.41±8.54)°in the control group,with statistically significant difference(t=7.149,7.644,P<0.05).After the intervention,the observation group’s eating score was(12.83±0.49)points,bathing score was(4.46±0.92)points,dressing score was(12.59±0.63)points,and grooming score was(11.89±0.74)points,which were higher than(10.68±1.54)points,(11.09±1.34)points,(10.13±1.87)points,and(10.03±1.02)points of the control group,and the differences were statistically significant(t=8.825,8.685,8.269,9.791,P<0.05).The incidence of complications in the observation group was 6.82%,which was lower than 22.73%in the control group,and the difference was statistically significant(χ^(2)=4.423,P<0.05).The total satisfaction rate of patients in the observation group was 95.45%,which was higher than 77.27%in the control group,and the difference was statistically significant(χ^(2)=6.175,P<0.05).Conclusion:The PDCA cycle management is effective in improving the recovery of hand function in patients with finger skin defects and flap repair,which is beneficial to reduce postoperative complications.
