内镜下钛夹配合注射肾上腺素在治疗老年溃疡性上消化道出血的效果观察

Application of endoscopic Adrenaline injection combined with Titanium clip in elderly patients with ulcerative upper gastrointestinal bleeding

目的:观察老年溃疡性上消化道出血患者治疗中应用内镜下钛夹配合肾上腺素注射的治疗效果。方法:选取我院2017年1月—2020年12月收治的88例老年溃疡性上消化道出血患者,采取随机数字表法将其分为常规组和观察组,各44例,常规组采取内镜下钛夹治疗,观察组采取内镜下钛夹配合注射肾上腺素进行治疗。对比两组治疗有效率、并发症发生率、治疗前后氧化应激指标、炎性因子指标、胃肠功能指标以及住院时间和输血情况。结果:观察组治疗总有效率高于常规组,差异有统计学意义(P<0.05);观察组并发症发生率低于常规组,差异有统计学意义(P<0.05);观察组氧化应激水平、炎性因子水平、胃肠激素水平均低于常规组,差异均有统计学意义(P<0.05);观察组住院时间短于常规组,输血率低于常规组,差异均有统计学意义(P<0.05)。结论:对老年溃疡性上消化道出血患者采取内镜下钛夹配合注射肾上腺素进行治疗,效果显著,可有效降低氧化应激、炎性因子水平、胃肠激素指标与并发症发生率,促进病情恢复、减少输血率,从而提高治疗效果。

Objective To observe the effect of endoscopic titanium clip combined with adrenaline injection in the treatment of elderly patients with ulcerative upper gastrointestinal bleeding.Methods 88 elderly patients treated in our hospital from January 2017 to December 2020 were selected as the research object.They were randomly divided into routine group and observation group,with 44 cases in each group.The routine group was treated with endoscopic Titanium clip,and the observation group was treated with endoscopic injection of Adrenaline combined with Titanium clip.The treatment effect,complication rate,oxidative stress index,inflammatory factor index,gastrointestinal function index,hospital stay and blood transfusion were compared between the two groups.Results the effective rate of the observation group was higher than that of the routine group,and the difference was statistically significant(P<0.05);The incidence of complications in the observation group was lower than that in the routine group(P<0.05);The levels of oxidative stress,inflammatory factors and gastrointestinal hormones in the observation group were lower than those in the routine group(P<0.05);The hospital stay in the observation group was shorter than that in the routine group,and the blood transfusion rate was lower than that in the routine group(P<0.05).Conclusion Endoscopic Adrenaline injection combined with Titanium clip can reduce the incidence of disease-related indicators and complications,promote the recovery of the disease and reduce the rate of blood transfusion,so as to improve the treatment effect.
