个体化控压注气法在经桡动脉行冠状动脉介入术后压迫止血中的应用效果 被引量:10

摘要 目的 探讨个体化控压注气法在经桡动脉行冠状动脉介入术(trans radial coronary intervention,TRI)术后压迫止血中的应用效果,以期为提高压迫止血质量提供临床依据。方法2019年6月至2020年6月,采用便利取样法选取某院行TRI术并使用气囊桡动脉压迫器止血患者为研究对象。按择期手术顺序分为观察组(141例)和对照组(139例)。观察组采用个体化控压注气法、对照组采用常规注气法压迫止血,比较止血效果、并发症(压力过大并发症、压力过小并发症)、末梢循环、疼痛程度。结果 两组患者压力过大并发症的差异有统计学意义(4.26%vs.13.67%,χ^(2)=7.628,P=0.006);两组患者压力过小并发症的差异无统计学意义(19.15%vs.15.83%,χ^(2)=0.535,P=0.465);两组疼痛程度、末梢循环差异均有统计学意义(均P<0.01);两组止血效果差异无统计学意义(P=0.317)。结论 TRI术后气囊桡动脉压迫器止血中使用个体化控压注气法,能有效压迫止血,减少并发症发生,减轻疼痛感。 Objective To explore the application effect of individualized pressure-controlled gas injection in hemostasis by compression after transradial coronary intervention(TRI)operation.Methods From June 2019 to June 2020,patients who underwent TRI and used balloon radial artery compression device to stop bleeding in a hospital were selected by convenience sampling method.According to the order of operation,the patients were divided into observation group(n=141)and control group(n=139).The observation group was treated with individual pressure-controlled gas injection,while the control group was treated with conventional gas injection for hemostasis.The hemostatic effect,complications(too much pressure complications,too little pressure complications),peripheral circulation and pain degree were compared between the two groups.Results There was a significant difference in the complications of stress between the two groups(4.26%vs.13.67%,χ^(2)=7.628,P=0.006).There was no significant difference in the complications of low stress between the two groups(19.15%vs.15.83%,χ^(2)=0.535,P=0.465).There were statistically significant differences in pain degree and peripheral circulation between the two groups(all P<0.01).There was no significant difference in hemostatic effect between the two groups(P=0.317).Conclusions Individualized pressure-controlled gas injection can effectively compress hemostasis,reduce complications,relieve pain in postoperative TRI hemostasis using radial artery balloon compression device.
