三焦点和双焦点人工晶状体植入术后视觉质量的比较研究

Comparative study of visual performance of tri-focal and double focal intraocular lens

目的比较两种多焦点人工晶状体(intraocular lens,IOL)植入术后的视功能。方法收集2018年4月至2019年3月在首都医科大学附属北京同仁医院进行白内障超声乳化联合多焦点IOL植入手术的40例患者(80只眼)。依据植入IOL的不同,分为三焦点IOL组(839MP组)和双焦点IOL组(ZMB00组),每组20例。在术后3个月比较植入两种IOL患者的未矫正远视力(uncorrected distance visual acuity,UDVA)、中视力(uncorrected intermediate visual acuity,UIVA)、近视力(uncorrected near visual acuity,UNVA),矫正远视力(corrected distance visual acuity,CDVA)、中视力(corrected intermediate visual acuity,CIVA)、近视力(corrected near visual acuity,CNVA),屈光度以及术后的满意度问卷调查。结果术后3个月两组患者之间UDVA、CDVA、CIVA、UNVA及CNVA间差异无显著性(P>0.05),839MP组的UIVA为0.14±0.16,ZMB00组的UIVA为0.30±0.23,839MP组的UIVA明显高于ZMB00组的UIVA(P=0.001)。839MP组中视力球镜度数为(0.35±0.93)D,ZMB00组的中视力球镜度数为(0.85±1.20)D,前者的中视力球镜度数显著小于后者(P=0.041)。两组的问卷调查在整体满意度、近距离阅读、夜间视力三个方面差异无显著性。在中距离视物方面839MP组的满意度明显高于ZMB00组。结论三焦点IOL能够给白内障术后患者提供有效的远、中、近全程视力,提供比双焦点IOL更好的中视力。

Objective To compare the visual function of two kinds of multifocal intraocular lens(IOL).Method Forty patients(80 eyes)who underwent cataract phacoemulsification combined with multifocal IOL implantation in Beijing Tongren Eye Center from April 2018 to 2019 were collected.According to the type of IOL,the patients were divided into two groups(tri-focal IOL 839MP and double focal IOL ZMB00)with 20 patients(40 eyes)in each group.Three months after operation,the uncorrected distance visual acuity(UDVA),uncorrected intermediate visual acuity(UIVA),uncorrected near visual acuity(UNVA),corrected distance visual acuity(CDVA),corrected intermediate visual acuity(CIVA)and corrected near visual acuity(CNVA),diopter and postoperative satisfaction questionnaire of patients with two kinds of IOLs were compared.Result Three months after operation,there was no significant difference between the two groups in UDVA,CDVA,CIVA,UNVA and CNVA(P>0.05).The UIVA of 839MP group was 0.14±0.16,and that of ZMB00 group was 0.30±0.23.The UNIVA of 839MP group was significantly higher than that of ZMB00 group(P=0.001).The spherical dioptor in 839MP group was(0.35±0.93)D,and was(0.85±1.20)D in ZMB00 group.The former was significantly lower than that of the latter(P=0.041).There was no significant difference in overall satisfaction,close reading and night vision between the two groups.The satisfaction on intermediate distance visual in 839MP group was significantly higher than that of ZMB00 group.Conclusion Triple focus IOL could provide effective distance,intermediate and near whole course vision for patients after cataract surgery,and could provide better intermediate visual acuity than bifocal IOL.
