螺内酯对高龄阵发性房颤并心力衰竭患者心功能的影响研究

The influence of spirolactone for cardiac function of the very elderly patients with paroxysmal atrial fibrillation and heart failure

目的观察螺内酯对高龄阵发性房颤(心房颤动)并心力衰竭患者心功能的影响。方法43例阵发性房颤合并心力衰竭患者,随机分为螺内酯治疗组(21例)和对照组(22例)。对照组接受基础治疗,螺内酯治疗组在对照组基础上接受螺内酯治疗。比较两组患者治疗前后氨基末端脑钠肽前体(NT-proBNP)、左心室射血分数(LVEF)和左心房内径(LAD)水平。结果治疗前,两组患者的NT-proBNP、LAD和LVEF水平比较差异均无统计学意义(P>0.05);治疗后,螺内酯治疗组患者的NT-proBNP(989.23±67.14)pg/ml低于对照组的(1063.62±67.32)pg/ml,LVEF(42.53±4.61)%高于对照组的(39.84±3.59)%,LAD(48.56±4.77)mm小于对照组的(51.23±3.26)mm,差异有统计学意义(P<0.05)。结论螺内酯可延缓高龄阵发性房颤并心力衰竭患者房颤及心力衰竭进展。

Objective To observe the influence of spirolactone for cardiac function of the very elderly patients with paroxysmal atrial fibrillation and heart failure.Methods A total of 43 patients with paroxysmal atrial fibrillation and heart failure were randomly divided into spironolactone treatment group(21 cases)and control group(22 cases).The control group received basic treatment,and the spironolactone treatment group received spironolactone treatment on the basis of the control group.The levels of N-terminal pro-brain natriuretic peptide(NT-proBNP),left ventricular ejection fraction(LVEF)and left atrial diameter(LAD)before and after treatment were compared between the two groups.Results Before treatment,there was no statistically significant difference in the levels of NT-proBNP,LAD and LVEF between the two groups(P>0.05).After treatment,the NT-proBNP(989.23±67.14)pg/ml of the spironolactone treatment group was lower than(1063.62±67.32)pg/ml of the control group,the LVEF(42.53±4.61)%was higher than(39.84±3.59)%of the control group,and the LAD(48.56±4.77)mm was smaller than(51.23±3.26)mm of the control group.All the differences were statistically significant(P<0.05).Conclusion Spirolactone delays the progress of atrial fibrillation and heart failure in elderly patients with paroxysmal atrial fibrillation and heart failure.
