摘要
高血糖和胰岛素抵抗或缺乏是糖尿病的特征,糖尿病伴随着能量代谢紊乱,而成人心脏收缩和舒张所需的能量主要由线粒体产生,糖尿病心肌病的心肌细胞线粒体受损,伴随着心肌细胞肥大、纤维化和凋亡,自噬尤其是具有高度特异性的线粒体自噬通过清除异常线粒体,在控制线粒体功能中起关键作用。尽管目前线粒体自噬在糖尿病心肌病进展中的作用尚未明确,但其在不同类型糖尿病心肌中的效果不同。因此,了解线粒体自噬在糖尿病心肌中的作用对于糖尿病心肌病预后、糖尿病住院患者临床管理以及潜在治疗方案均有重要意义。
Hyperglycemia and insulin resistance or deficiency are characteristics of diabetes mellitus.Long-term disturbances in energy metabolism exist in diabetes mellitus,and the energy required for contraction and diastole of the adult heart is mainly produced by mitochondria.In diabetic cardiomyopathy,mitochondria within the cardiomyocyte are damaged,accompanied by cardiomyocyte hypertrophy,fibrosis and apoptosis.Autophagy,especially mitochondrial autophagy with high specificity,plays a key role in controlling mitochondrial function by removing abnormal mitochondria.Although the role of mitochondrial autophagy in the progression of diabetic cardiomyopathy is not yet understood,research findings have shown its multiple effects in different types of diabetic hearts.Therefore,understanding the role played by mitochondrial autophagy in the diabetic myocardium has significant implications for the prognosis of diabetic cardiomyopathy,the clinical management of diabetic inpatients,and potential treatment options.
