眼眶高分化脂肪肉瘤1例

A case of orbital Well-Differentiated liposarcoma

43岁女性,因“右眼球突出8个月余,眼红、眼痛2个月”入院。体格检查发现右眼球突出,外转不能、余方位轻度受限,上睑轻度水肿,球结膜高度充血水肿。全身伴随甲亢症状、甲状腺功能与甲状腺抗体异常,影像学显示右眼单纯外直肌显著增粗。入院后予右眼眶占位完整切除,病理提示非典型性脂肪瘤样肿瘤/高分化脂肪肉瘤。讨论体会:眼外观情况、眼外肌增粗与眼球运动情况的对应、影像学细节在眼眶占位性病变诊断中尤为重要。

A 43-year-old female presented with an 8-month history of proptosis,2-month of redness and pain in the right eye.Ocular examination revealed proptosis,abduction function loss and slightly restricted movement in other directions,swelling of the upper eyelid,obvious conjunctival hyperemia and edema in the right eye.The patient also had hyperthyroidism symptoms,and abnormalities of thyroid function and thyroid antibody revealed by blood test.Imageological examinations demonstrated an extreme enlargement of lateral rectus.Thus,an orbital tumorectomy was performed,and the diagnosis of atypical lipomatous Tumor/Well-Differentiated liposarcoma was confirmed by histopathological examination.In conclusion,oular appearance,the relationship between extraocular muscle thickening and eye movement,and details of imaging manifestations are key points in the diagnosis of orbital space-occupying lesions.
