过早搏动中医证候与合并病的相关性分析

Correlation Analysis Between Syndrome of Premature Beats and Complicated Diseases

目的:探讨过早搏动的中医证候分布规律及与合并病的相关性,为过早搏动的临床辨证施治提供依据。方法:本研究纳入河南省中医院2010年5月至2020年1月经确诊的1849名过早搏动患者,利用聚类分析、因子分析、对应分析方法,探索过早搏动中医证候与合并病的相关性。结果:过早搏动常见中医证候为痰火扰心证、阴虚火旺证、气阴两虚瘀热互结证、心血瘀阻证、心脾两虚证、心阳不振证等,常见合并病为冠心病、高血压、糖尿病、高脂血症、心力衰竭,其中糖尿病与阴虚火旺证、气阴两虚瘀热互结证关系密切,高血压、高脂血症与心脾两虚证、痰火扰心证关系密切,冠心病、心力衰竭与心血瘀阻证、心阳不振证关系密切。结论:不同合并病的过早搏动中医证候分布不同,在临床治疗中结合不同的证候特点辨证施治。

Objective:To explore the regularity of syndrome distribution of premature beats and its correlation with complicated diseases,so as to provide evidence for clinical syndrome differentiation and treatment of premature beats.Methods This study has been included in 1849 patients with premature beats diagnosed in Henan Provincial Hospital of Traditional Chinese Medicine from May 2010 to January 2020.By means of cluster analysis,factor analysis and correspondence analysis,the correlation between premature beats syndrome and complicated diseases was explored.Results:The common syndromes of premature beat were phlegm-fire disturbing heart,Yin deficiency and fire hyperactivity,Qi-Yin deficiency,blood stasis,heart-spleen deficiency,heart-Yang deficiency,etc.Common complications are coronary heart disease,hypertension,diabetes,hyperlipidemia and heart failure.Among them,diabetes is closely related to hyperactivity of fire due to Yin deficiency,stagnation of blood stasis and heat due to deficiency of both Qi and Yin,hypertension and hyperlipidemia are closely related to deficiency of both heart and spleen,phlegm-fire disturbing heart,coronary heart disease and heart failure are closely related to heart blood stasis and heart Yang deficiency.Conclusion:The distribution of syndromes of premature beat of different combined diseases is different,and different syndrome characteristics should be combined in clinical treatment.
