消化专科医师幽门螺杆菌感染处理现状调查及培训

Investigation of management of Helicobacter pylori infection by gastroenterologists and application of professional training

目的 探讨消化专科医师幽门螺杆菌(Helicobacter pylori,H.pylori)感染处理现状及专业培训的效果。方法 自2021年5月至2022年2月对中国人民解放军总医院第一医学中心和中国人民解放军总医院海南医院58名消化专科医师开展问卷调查,了解医师对H.pylori感染处理现状。通过专题讲座和临床教学对医师进行培训,以根除治疗处方合格率和根除成功率评估培训效果。结果 共收集有效问卷58份。受访者根除H.pylori的方案准确率为51.72%(30/58)。培训前根除方案中处方合格率为89.39%(1 239/1 386),培训后处方合格率为100%(1 052/1 052),合格率提高10.61%(89.39%vs 100%,P=0.000)。根除成功率提高了9.85%(79.55%vs 89.40%,P=0.001)。结论 消化专科医师对H.pylori感染的规范处理仍不乐观,系统培训对指导医师规范处理H.pylori感染、避免随机治疗、提高初次根除率有明显效果。

Objective To investigate the management of Helicobacter pylori(H.pylori) infection by gastroenterologists and effects of professional training.Methods Questionnaire survey about knowledge of diagnosis and treatment of H.pylori and training were conducted in 58 gastroenterologists in the First Medical Center,Chinese PLA General Hospital and Hainan Hospital,Chinese PLA General Hospital.The training effect was evaluated by prescription qualification rate and eradication rate after training.Results A total of 58 valid questionnaires were collected.Choice accuracy of eradication regimens was 51.72%(30/58) among the respondents.After training,all the prescriptions were qualified(89.39% vs 100%,P=0.000) and the eradication rate of H.pylori increased by 9.85%(79.55% vs 89.40%,P=0.001).Conclusion Systematic training was helpful to guide gastroenterologists to treat H.pylori infection professionally,avoiding voluntary treatment and increase primary eradication rate.
