不同类型胸腔镜治疗非小细胞肺癌的临床效果及对患者并发症的影响

Clinical effect of different types of thoracoscopy in the treatment of non-small cell lung cancer and its impact on complications

目的探讨不同类型胸腔镜治疗非小细胞肺癌的临床效果及对患者并发症的影响。方法选取2018年1月至2021年1月我院收治的120例非小细胞肺癌患者为研究对象,随机编号后按照奇偶号分为单孔组和三孔组,每组60例。单孔组实施单孔胸腔镜肺叶切除术加纵隔淋巴结清扫术,三孔组实施三孔胸腔镜肺叶切除术加纵隔淋巴结清扫术。比较2组临床指标、术后疼痛程度、淋巴结清扫个数以及术后并发症发生率。结果单孔组手术时间长于三孔组,术中出血量低于三孔组,胸管留置时间及住院时间均短于三孔组(P<0.05)。单孔组术后1 d、术后3 d、术后7 d疼痛视觉模拟评分(VAS)评分均低于三孔组(P<0.05)。2组淋巴结清扫个数与站数、N2淋巴结清扫个数与站数比较差异均无统计学意义(P>0.05)。单孔组术后并发症总发生率为8.3%,三孔组为11.7%,但2组比较差异无统计学意义(P>0.05)。结论单孔胸腔镜手术治疗非小细胞肺癌的临床效果显著,有利于缓解患者术后疼痛程度,促进术后恢复,值得临床实践应用。

Objective To explore the clinical effect of different types of thoracoscopy in the treatment of non-small cell lung cancer and its impact on patients′complications.Methods One hundred and twenty patients with non-small cell lung cancer treated in our hospital from January 2018 to January 2021 were randomly divided into single well group and three well group according to parity sequence,with 60 cases in each group.Single hole thoracoscopic lobectomy plus mediastinal lymph node dissection was performed in the single well group,and three hole thoracoscopic lobectomy plus mediastinal lymph node dissection was performed in the three well group.The clinical indexes,postoperative pain,the number of lymph node dissections and the incidence of postoperative complications were compared between the two groups.Results In the single well group,the operation time was longer,the intraoperative blood output was lower,and the retention time of chest tube and hospital stay were shorter than those in the three well group(P<0.05).At 1 day,3 days and 7 days after operation,the scores of visual analogue scales(VAS)in the single well group were lower than those in the three well group(P<0.05).There were no significant differences in the number of lymph node dissections and distribution location,N2 lymph node dissections and stations between the two groups(P>0.05).The total incidence of postoperative complications in the single well group was 8.3%,11.7%in the three well group,but there was no significant difference between the two groups(P>0.05).Conclusion The clinical effect of single well thoracoscopic surgery in the treatment of non-small cell lung cancer is significant,which is conducive to alleviate the degree of postoperative pain and promote postoperative recovery.It is worthy of clinical practice.
