彩色多普勒血流成像对鼻咽癌放化疗后颈动脉狭窄的早期诊断价值研究

The value of color doppler flow imaging in the early diagnosis of carotid artery stenosis after radiotherapy and chemotherapy for nasopharyngeal carcinoma

目的:研究彩色多普勒血流成像(CDFI)对鼻咽癌(NPC)放化疗后颈动脉狭窄的早期诊断价值。方法:选择2018年1月—2019年12月于赣南医学院第一附属医院行放化疗的70例NPC患者作为放疗组,选取同期在该院确诊因各种原因而未行放疗而单纯行化疗的70例NPC患者作为对照组。随访2年,两组均接受CDFI检查。比较两组颈动脉平均血流参数、颈动脉内径及颈动脉狭窄、颈动脉斑块形成发生率。结果:放疗组颈动脉峰值收缩期血流流速(PSV)、平均流速(Vm)、搏动指数(PI)、阻力指数(RI)高于对照组,血管内径小于对照组,差异有统计学意义(P<0.05);放疗组斑块形成率、颈动脉狭窄发生率高于对照组,差异有统计学意义(P<0.05)。结论:NPC放化疗会造成颈动脉损伤,导致颈动脉狭窄。CDFI检查能够测定颈动脉血管内径及血流动力学参数,为诊断颈动脉狭窄提供依据。

Objective:To study the value of color Doppler flow imaging(CDFI)in the early diagnosis of carotid artery stenosis after radiotherapy and chemotherapy for nasopharyngeal carcinoma(NPC).Methods:All 70 NPC patients who received chemotherapy and radiotherapy in the First Affiliated Hospital of Gannan Medical College from January 2018 to December 2019 were selected as the radiotherapy group,and 30 NPC patients who were diagnosed with chemotherapy alone and radiotherapy was not performed for various reasons in the hospital during the same period were selected as the control group.Followed up for 2 years,both groups received CDFI examination.The average blood flow parameters of the carotid arteries and the internal diameter of the carotid arteries were compared between the two groups,as well as the incidence of carotid artery stenosis and carotid plaque formation in the two groups.Results:Carotid artery peak systolic blood flow velocity(PSV),average velocity(Vm),pulsatility index(PI),resistance index(RI)higher than the control group,blood vessel inner diameter smaller than the control group,there was a statistical difference(P<0.05).The plaque formation rate and the incidence of carotid artery stenosis in the radiotherapy group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:NPC radiotherapy and chemotherapy can cause carotid artery damage and cause carotid artery stenosis.CDFI can determine the carotid artery diameter and hemodynamic parameters,and provide a basis for the diagnosis of carotid artery stenosis.
