HPV试剂盒与宫颈液基薄层细胞学检查法在宫颈癌和癌前病变早期筛查中的应用效果比较

Comparison of application effects of human papillomavirus(HPV)kit and cervical thin prep cytologic test in early screening of cervical cancer and precancerous lesions of uterine cervix

目的:比较凯普人乳头瘤病毒(HPV)试剂盒与宫颈液基薄层细胞学检查法(TCT)在宫颈癌和癌前病变早期筛查中的应用效果。方法:选取于该院就诊并自愿进行HPV筛查的1896例患者为研究对象,均进行凯普HPV试剂盒检测、TCT检测和组织病理检查,以组织病理检查结果为金标准,比较凯普HPV试剂盒检测与TCT检测在宫颈上皮内瘤变(CIN)和宫颈癌检测中的阳性检出率。结果:组织病理检查结果显示,1896例HPV筛查患者中,CIN1级34例,CIN2级42例,CIN3级23例,宫颈癌10例;凯普HPV试剂盒检测CIN1级阳性检出率为73.53%(25/34),高于TCT检测的61.76%(21/34);CIN2级阳性检出率为90.48%(38/42),高于TCT检测的64.29%(27/42);CIN3级阳性检出率为91.30%(21/23),高于TCT检测的69.57%(16/23);宫颈癌阳性检出率为100.00%(10/10),高于TCT检测的80.00%(8/10)。结论:凯普HPV试剂盒检测在CIN和宫颈癌患者中的应用效果优于TCT检测。

Objective:To compare application effects of human papillomavirus(HPV)kit and cervical thin prep cytologic test(TCT)in early screening of cervical cancer and precancerous lesions of uterine cervix.Methods:1896 patients who visited the hospital and voluntarily underwent HPV screening were selected as the research subjects,and all of them were examined by Kapp HPV kit and TCT.Histopathological examinations were performed on the subjects with positive HPV kit results and the TCT results≥atypical squamous cells or negative HPV and TCT results≥low-grade squamous intraepithelial lesions.Taking the results of histopathological examination as the gold standard,the positive detection rates of HPV kit test and TCT for cervical intraepithelial neoplasia(CIN)and cervical cancer were compared.Results:The results of histopathological examination showed that among the 1896 HPV screening patients,34 with CIN1,42 with CIN2,23 with CIN3,and 10 with cervical cancer.The positive detection rate of CIN1 detected by Kapp HPV kit was 73.53%(25/34),which was higher than 61.76%(21/34)of TCT;the positive detection rate of CIN2 was 90.48%(38/42),which was higher than 64.29%(27/42)of TCT;the positive detection rate of CIN3 was 91.30%(21/23),which was higher than 69.57%(16/23)of TCT;and the positive detection rate of cervical cancer was 100.00%(10/10),which was higher than 80.00%(8/10)of TCT.Conclusions:The Kapp HPV kit is more effective than TCT in the detection of the patients with CIN and cervical cancer.
