双极人工股骨头置换术与全髋关节置换术治疗老年移位股骨颈骨折患者的效果比较

Comparison of effects of bipolar artificial femoral head replacement and total hip arthroplasty in treatment of elderly patients with displaced femoral neck fractures

目的:比较双极人工股骨头置换术与全髋关节置换术治疗老年移位股骨颈骨折患者的效果。方法:回顾性分析2020年该院收治的62例老年移位股骨颈骨折患者的临床资料,根据手术方式不同分为A组(行双极人工股骨头置换术)与B组(行全髋关节置换术)各31例。比较两组髋关节功能优良率、手术相关指标水平和并发症发生率。结果:两组髋关节功能优良率比较,差异无统计学意义(P>0.05);A组手术时间短于B组,术后引流量、术中出血量均少于B组,差异有统计学意义(P<0.05);两组住院时间比较,差异无统计学意义(P>0.05);B组并发症发生率为6.45%(2/31),低于A组的25.81%(8/31),差异有统计学意义(P<0.05)。结论:双极人工股骨头置换术与全髋关节置换术治疗老年移位股骨颈骨折患者的髋关节功能优良率和住院时间相当,但双极人工股骨头置换术的手术时间短于全髋关节置换术,术后引流量、术中出血量均少于全髋关节置换术,而全髋关节置换术的并发症发生率低于双极人工股骨头置换术。

Objective:To compare effects of bipolar artificial femoral head replacement and total hip arthroplasty in treatment of elderly patients with displaced femoral neck fractures.Methods:The he clinical data of 62 elderly patients with displaced femoral neck fractures admitted to our hospital in 2020 were retrospectively analyzed.According to different surgical methods,they were divided into group A(bipolar artificial femoral head replacement)and group B(total hip arthroplasty),31 cases in each group.The excellent and good rate of hip function,the levels of surgery-related indicators and the incidence of complications were compared between the two groups.Results:There was no significant difference in the excellent and good rate of hip function between the two groups(P>0.05).The operation time of group A was shorter than that of group B;the postoperative drainage volume and the intraoperative blood loss were less than those of group B;and the differences were statistically significant(P<0.05).There was no significant difference in the hospitalization time between the two groups(P>0.05).Further,the complication rate of group B was 6.45%(2/31),which was lower than group A of 25.81%(8/31),and the difference was statistically significant(P<0.05).Conclusions:Bipolar artificial femoral head arthroplasty and total hip arthroplasty for the elderly patients with displaced femoral neck fractures have comparable excellent and good rate of hip function and the hospitalization time.The operation time of bipolar artificial femoral head replacement is shorter than that of total hip replacement;the postoperative drainage volume and the intraoperative blood loss are less than those of total hip arthroplasty;however,the complication rate of total hip arthroplasty is lower than that of artificial femoral head replacement.
