中医外治法路径化干预对马蹄形肛周脓肿患者术后快速康复的影响

Influence of perioperative external therapy of Traditional Chinese Medicine path-based intervention on rapid postoperative recovery of patients with horseshoe shaped perianal abscess

目的 探讨围术期中医外治法路径化干预对马蹄形肛周脓肿患者术后快速康复的影响。方法 选取2018年9月至2019年12月浙江中医药大学附属嘉兴市中医医院门诊确诊收治入院的马蹄形肛周脓肿(肛管后深间隙)患者60例,随机分为观察组与对照组,每组各30例。观察组采用路径化的中医外治法和西医常规方法,对照组采用西医常规方法。比较两组术后疼痛评分、二便情况、住院时间、创面愈合时间和临床疗效等。结果 观察组术后第1、3、7天的视觉模拟量表(VAS)评分低于对照组(P<0.05),观察组术后尿潴留和便秘情况发生率明显低于对照组(P<0.05),观察组的住院时间、创面愈合时间短于对照组(P<0.05)。两组的肛门功能Wexner评分及临床疗效比较,差异无统计学意义(P>0.05)。结论 路径化的中医外治法有序、适时及有针对性的干预,可有效减少和缓解术后并发症,提高患者的生活质量,促进术后快速康复。

Objective To explore the influence of perioperative external therapy of Traditional Chinese Medicine pathbased intervention on the rapid postoperative recovery of patients with horseshoe shaped perianal abscess. Methods A total of 60 patients with horseshoe shaped perianal abscess(deep posterior anal space) who were diagnosed in and admitted to the Department of Outpatient of our hospital from September 2018 to December 2019 were selected and randomly divided into the observation group and the control group, with 30 patients in each group. The observation group was treated with the path-based external therapy of Traditional Chinese Medicine and western medicine routine method, and the control group was treated with the western medicine routine method. The postoperative pain score,defecation and urination, length of hospital stay, wound healing time and clinical efficacy were compared between the two groups. Results The visual analogue scale(VAS) score of the observation group was lower than that of the control group(P<0.05). The incidences of urinary retention and constipation in the observation group were significantly lower than those of the control group(P<0.05). The length of hospital stay and wound healing time of the observation group were shorter than those of the control group(P<0.05). No significant differences were observed in the Wexner score of anal function and clinical efficacy between the two groups(P>0.05). Conclusion Orderly, timely and targeted intervention of path-based external therapy of Traditional Chinese Medicine can effectively reduce and alleviate postoperative complications, improve the quality of life of patients, and promote rapid postoperative recovery.
