摘要 目的 分析肝癌微波消融治疗中采用肝脏实时超声造影(CEUS)的价值。方法 选择2019年4月至2021年6月湖南省娄底市中心医院住院部超声科接收肝癌微波消融治疗患者50例,共治疗78个病灶,在实施微波消融治疗前、治疗中、治疗后均实施肝脏超声造影检查,其中35例肝癌患者采用经皮穿刺肝脏肿瘤微波消融治疗(观察组),15例肝癌患者采用B超导向微波消融治疗(对照组),分析50例患者治疗后并发症率、不同方案准确率、生存率。结果 50例肝癌患者治疗后并发症率为0(0/50);观察组35例经治疗后显效64.11%、有效33.33%、无效2.56%,CEUS评估观察组治疗后疗效准确率为97.44%(76/78);对照组15例经治疗后显效51.28%、有效23.08%、无效25.64%,CEUS评估对照组治疗后疗效准确率为74.36%(58/78)(P<0.05);观察组6、12个月生存率高于对照组。结论 选择肝脏实时超声造影对肝癌微波消融治疗效果进行评估准确率较高,可作为评估随访结果方案,亦可为临床医师治疗提供丰富影像学参考,值得借鉴。 Objective To analyze the value of real-time contrast ultrasound technology(CEUS) in microwave ablation for liver cancer. Methods A total of 50 patients treated with microwave ablation for liver cancer in the Ultrasound Department of the Inpatient Department of Loudi Central Hospital in Hunan Province from April 2019 to June 2021were selected as the research subjects, and a total of 78 lesions were treated. The contrast ultrasound was performed before, during, and after the microwave ablation. 35 patients with liver cancer were treated with percutaneous puncture microwave ablation for liver tumor(the observation group), and 15 patients with liver cancer were treated with B-ultrasound-guided microwave ablation(the control group). The complication rate, accuracy rate and survival rate of different regimens after treatment were analyzed in 50 patients. Results The complication rate of 50 patients with liver cancer after treatment was 0(0/50);after treatment, 35 cases in the observation group were markedly effective in64.11%, effective in 33.33% and ineffective in 2.56%, and the CEUS assessment of the observation group after treatment showed an accuracy of 97.44%(76/78);after treatment, 15 cases in the control group were markedly effective in 51.28%, effective in 23.08%, and ineffective in 25.64%, and the CEUS assessment of the control group after treatment was effective in 74.36%(58/78)(P<0.05);the 6-month and 12-month survival rates of the observation group were higher than those of the control group. Conclusion Liver real-time contrast ultrasound technology has high accuracy in evaluating the effect of microwave ablation on liver cancer. It can be used as a regimen for evaluating the results of follow-up, and it can also provide rich imaging references for clinicians in treatment. Therefore, it is worthy of reference.
