磁共振扩散加权成像对布鲁杆菌性脊柱炎的临床价值

摘要 目的:探讨磁共振扩散加权成像对布鲁杆菌性脊柱炎的诊断价值。方法:选取2015年1月—2020年8月河南大学第一附属医院诊治的40例布鲁杆菌性脊柱炎患者,回顾分析临床资料,分别行常规CT、磁共振成像(MRI)、磁共振扩散加权成像(DWI)检查,总结病变椎体检出率,分析影像学特点。结果:CT对布鲁杆菌性脊柱炎病变椎体检出率为70.00%,MRI检出率为82.50%,DWI技术检出率97.50%,差异有统计学意义(χ^(2)=66.692、30.000,P<0.05)。MRI技术对椎旁、腰大肌内、椎管内软组织影及腰背部软组织肿胀检出率高于CT,颈椎、胸椎、腰椎慢性期ADC值均低于急性期、亚急性期,差异有统计学意义(F=10.240、5.443、3.403,P<0.05)。结论:磁共振扩散加权成像对布鲁杆菌性脊柱炎有一定检出价值,DWI表现特征性明显。 Objective: To investigate the diagnostic value of magnetic resonance diffusion weighted imaging for brucellosis spondylitis. Methods: Forty patients with brucellosis spondylitis diagnosed and treated in the hospital from January 2015 to August2020 were selected. The clinical data were retrospectively analyzed, and routine CT, magnetic resonance imaging(MRI), and diffusion-weighted magnetic resonance imaging(DWI) were performed. The detection rate of diseased vertebrae was summarized and the imaging characteristics were analyzed. Results: The CT detection rate of brucellosis spondylitis diseased vertebrae was 70.00%,MRI detection rate was 82.50%, DWI technique detection rate was 97.50%, the difference was statistically significant(χ^(2)=66.692,30.000, P<0.05). The detection rate of MRI technology for paravertebral, psoas muscle, intraspinal soft tissue shadow and lumbar and back soft tissue swelling was higher than CT. The ADC values of cervical spine, thoracic spine and lumbar spine in chronic phase were lower than those in acute and subacute phases, and the difference was statistically significant(F=10.240, 5.443, 3.403,P<0.05). Conclusion: Diffusion-weighted magnetic resonance imaging has a certain value in the detection of Brucella spondylitis,and DWI has obvious characteristics.
