白细胞介素、肿瘤坏死因子在重症患者应激性溃疡中的表达水平

Expression levels of interleukin and tumor necrosis factor in severe patients with stress ulcer

目的探讨白细胞介素、肿瘤坏死因子在重症患者应激性溃疡中的表达水平,以期为重症患者并发应激性溃疡的预防措施和治疗方案的制订提供参考依据。方法采用回顾性分析的方法,选取2019年1月至2020年6月浙江省台州医院收治的重症患者218例,基于其有无并发应激性溃疡进行分组研究,将发生应激性溃疡的重症患者设定为观察组(n=33),未发生应激性溃疡的重症患者设定为对照组(n=185),对两组患者的APACHEⅡ评分、白细胞介素及肿瘤坏死因子等表达水平进行统计分析。结果纳入的重症患者中应激性溃疡发生率为15.14%;观察组患者的APACHEⅡ评分高于对照组患者,差异有统计学意义(P<0.05);观察组患者的血清IL-2表达水平低于对照组,而IL-6及TNF-α水平高于对照组,差异均有统计学意义(P<0.05)。结论应激性溃疡作为重症患者最为严重的并发症之一,具有较高的病死率,当机体处于应激状态下其白细胞介素-2则出现降低的趋势,应激性溃疡越严重,白细胞介素-6及TNF-α表达水平则越高,许多细胞因子在应激性溃疡的发生发展中发挥重要作用。

Objective To investigate the expression levels of interleukin and tumor necrosis factor in severe patients with stress ulcers,to provide a reference for the prevention and treatment.Methods A retrospective analysis of 218 severe patients admitted to Taizhou Hospital of Zhejiang Province affiliated to Wenzhou Medical University from January 2019 to June 2020 was conducted.According to whether they had complicated,they were divided into the observation group(n=33)of patients with and the control group(n=185)with patients without stress ulcers.The APACHEⅡ scores,and expression levels of interleukin and tumor necrosis factor in the two groups were statistically analyzed.Results The incidence of stress ulcer among the included severe patients was 15.14%.The APACHEⅡ score in the observation group was higher than that in the control group,with statistically significant difference(P<0.05).The expression level of serum IL-2 in the observation group was lower than that in the control group,while the expression levels of IL-6 and TNF-α in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).Conclusion Stress ulcer is one of the most serious complications in severe patients,with high mortality rate.When the body is under stress,the level of IL-2 tends to decrease.The more serious the stress ulcer,the higher the levels of IL-6 and TNF-α.Many cytokines play an important role in the onset and development of stress ulcers.
