八髎穴穴位贴敷联合消痈汤对高位肛周脓肿术后疼痛及创面状态、炎症因子的影响

Effects of Baliao acupoint application and Xiaoyongtang combined treatment on postoperative pain,wound state and inflammatory factors of high perianal abscess

目的探讨八髎穴穴位贴敷联合消痈汤对高位肛周脓肿术后疼痛及创面状态、炎症因子的影响。方法选取2018年4月至2020年4月在浙江省丽水市中医院治疗的98例高位肛周脓肿患者,随机分为两组,每组各49例。对照组术后给予消痈汤治疗,治疗组给予八髎穴穴位贴敷联合消痈汤治疗,疗程2周。比较两组的疗效、肛周疼痛评分(visual analogue scale,VAS)、创面状态、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、C-反应蛋白(C-reaction protein,CRP)、白介素-1β(interleukin-1β,IL-1β)、白介素-6(interleukin-6,IL-6)。结果治疗组总有效率97.96%,高于对照组的85.71%,差异有统计学意义(P<0.05)。治疗组治疗3、7、14 d时的VAS评分均低于对照组,差异有统计学意义(P<0.05)。治疗组治疗后的创面水肿、渗出评分低于对照组,且治疗组的创面腐肉脱落、水肿消失、渗液消失时间及创面愈合时间均短于对照组,差异有统计学意义(P<0.05)。治疗后,治疗组的TNF-α、CRP、IL-1β、IL-6水平均低于对照组,差异有统计学意义(P<0.05)。结论八髎穴穴位贴敷联合消痈汤配合手术治疗高位肛周脓肿可有效减轻术后肛周疼痛,改善创面状态,促进创面愈合,降低炎症水平,提高临床疗效。

Objective To investigate the effects of Baliao acupoint application and Xiaoyongtang combined treatment on postoperative pain,wound state and inflammatory factors of high perianal abscess.Methods A total of 98 patients with high perianal abscess treated in Lishui Hospital of Traditional Chinese Medicine in Zhejiang Province from April 2018 to April 2020 were selected.They were randomly divided into two groups with 49 patients in each group.The control group was treated with Xiaoyongtang after operation,and the treatment group was treated with Baliao acupoint application combined with Xiaoyongtang for 2 weeks.The efficacy,perianal pain score(VAS),wound status,levels of inflammatory factors[tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),interleukin-1β(IL-1β),interleukin-6(IL-6)]were compared between the two groups.Results The total effective rate in the treatment group(97.96%)was higher than that in the control group(85.71%)(P<0.05).The VAS scores of the treatment group were lower than those of the control group at 3,7 and 14 days after treatment(P<0.05).After treatment,the scores of wound edema and exudation in the treatment group were lower than those in the control group,and the time of wound decaying,edema disappearance,exudation disappearance and wound healing time in the treatment group were shorter than those in the control group,with statistically significant differences(P<0.05).After treatment,the levels of TNF-α,CRP,IL-1β and IL-6 in the treatment group were lower than those in the control group,with statistically significant differences(P<0.05).Conclusion The treatment of high perianal abscess with Baliao acupoint application and Xiaoyongtang combined with surgery can effectively relieve postoperative perianal pain,improve the wound state,promote the wound healing,reduce the inflammation level and improve clinical efficacy.
