乳腺区段切除术对非哺乳期乳腺炎的疗效研究

Effect of segmental mastectomy on non-puerperal mastitis

目的观察乳腺区段切除术对非哺乳期乳腺炎(NPM)患者的疗效。方法选择2020年1月至2021年1月湖南省娄底市中心医院收治的且经超声检查+病理确诊的NPM患者80例,随机分为观察组(40例,行乳腺区段切除术)与对照组(40例,行保守治疗),评价组间血清炎性因子[白细胞介素-18(IL-18)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、治疗时间、外观满意度、复发率、乳房美容优良率、并发症发生率、临床有效率。结果①治疗前,组间IL-18、IL-6、TNF-α比较,差异无统计学意义(P>0.05);治疗后,观察组的IL-18、IL-6、TNF-α水平较低(P<0.05);②观察组的病情迁延时间[(37.78±7.24)d]、住院时间[(9.36±1.22)d]均短于对照组[(109.59±18.88)d、(11.58±1.64)d],外观满意度(82.50%)高于对照组(60.00%),复发率(0)低于对照组(10.00%)(P<0.05);③观察组的乳房美容优良率(95.00%)、临床有效率(95.00%)均高于对照组(77.50%、75.00%),并发症发生率(7.50%)低于对照组(32.50%)(P<0.05)。结论对NPM患者行乳腺区段切除术,可尽快减轻炎症状态,增强疗效,缩短康复进程,降低复发率,减少并发症,提高乳房美容优良率及外观满意度,值得推广。

Objective To observe the therapeutic effect of segmental mastectomy on non-puerperal mastitis(NPM).Methods A total of 80 NPM patients who were admitted from January 2020 to January 2021 in Loudi Central Hospital in Hunan Province and confirmed by ultrasound and pathology were selected and randomly divided into the observation group(40 patients were included and given segmental mastectomy)and the control group(40 patients were included and given conservative treatment).The serum inflammatory factors(interleukin-18[IL-18],interleukin-6[IL-6],tumor necrosis factor-α[TNF-α]),treatment time,appearance satisfaction,recurrence rates,breast beauty rates,complication rates,clinical effective rates were evaluated in the 2 groups.Results①Before treatment,no statistically significant differences were observed in IL-18,IL-6 and TNF-αbetween groups(P>0.05);after treatment,IL-18,IL-6 and TNF-α were lower in the observation group(P<0.05).②The duration of disease in the observation group([37.78±7.24]d)and the length of hospital stay([9.36±1.22]d)were less than those in the control group([109.59±18.88]d and[11.58±1.64]d);the appearance satisfaction(82.50%)was higher than that in the control group(60.00%);the recurrence rate(0)was lower than that in the control group(10.00%)(P<0.05).③The breast beauty rate(95.00%)and clinical effective rate(95.00%)in the observation group were higher than those in the control group(77.50% and 75.00%),and the complication rate(7.50%)was lower than that in the control group(32.50%)(P<0.05).Conclusion Segmental mastectomy for NPM patients can reduce the inflammatory state as soon as possible,enhance the efficacy,shorten the rehabilitation process,reduce the recurrence rate,reduce complications,and improve the rate of breast beauty and appearance satisfaction,which is worth promoting.
