妇科急腹症经腹和经阴道超声诊断及误诊分析

Diagnosis and misdiagnosis analysis of transabdominal and transvaginal ultrasound

目的:探讨经腹部和经阴道超声诊断急腹症的价值及超声声像图特点。方法:回顾性分析2020年4月—2021年5月安徽省妇幼保健院收治的120例急腹症患者经腹部和经阴道超声检查结果,比较两种检查的准确率。结果:病理结果是异位妊娠78例,经阴道和经腹超声诊断准确率分别为85.9%、64.1%;黄体破裂18例,经阴道和经腹超声诊断准确率分别为83.3%、66.7%;输卵管卵巢脓肿10例,经阴道和经腹超声诊断准确率分别为90%、80%;卵巢囊肿蒂扭转14例,经阴道和经腹超声诊断准确率均为100%。结论:经阴道超声妇科急腹症的诊断与鉴别诊断具有较高的准确率,并优于经腹部超声。

Objective To investigate the value of transabdominal and transvaginal ultrasound and its characteristics.Methods 120 patients with acute abdomen admitted to Anhui Province Maternity and Child Health Hospital from April 2020 to May 2021 were retrospectively analyzed by transabdominal and transvaginal ultrasonography,and the accuracy of the two examinations was compared.Results The pathological results showed that 78 cases of ectopic pregnancy were diagnosed by transvaginal ultrasonography and transabdominal ultrasonography.The diagnostic accuracy was 85.9%and 64.1%,respectively.The pathological results showed that 18 cases of corpus luteum rupture,the diagnostic accuracy of transvaginal and transabdominal ultrasonography was 83.3%and 66.7%,respectively.The pathological results showed that 10 cases of tubo-ovarian abscess,the diagnostic accuracy of transvaginal and transabdominal ultrasonography was 90%and 80%,respectively.14 cases of torsion of ovarian cyst pedicle were diagnosed with 100%accuracy by transvaginal and transabdominal ultrasonography.Conclusion The diagnosis and differential diagnosis of transvaginal ultrasound has high accuracy and is significantly better than transabdominal ultrasound.
