干扰素和直接作用抗病毒药物治疗慢性丙型肝炎所致抑郁症发病机制研究进展

Research progress on pathogenesis of depression caused by interferon and direct acting antiviral drugs in treatment of chronic hepatitis C

丙型肝炎病毒(HCV)感染可导致急、慢性肝炎,进而发展为肝硬化或肝癌。绝大多数丙型病毒性肝炎是可以治愈的,治疗有效措施包括聚乙二醇长效干扰素和直接作用抗病毒药物(DAAs)。但既往研究发现,患者在干扰素治疗期间可发生抑郁症,相关机制可能与下丘脑-垂体-肾上腺(HPA)轴活性异常、脑衍生神经营养因子假说、多不饱和脂肪酸(PUFAs)紊乱、炎症学说、特定基因的单核苷酸多态性(SNP)以及细胞因子学说有关;此外,抑郁症在DAAs治疗中也可能发生。现对干扰素和DAAs治疗慢性丙型肝炎相关抑郁症发病机制的研究进展进行综述。

Hepatitis C virus(HCV)infection can lead to acute and chronic hepatitis,and then develop into liver cirrhosis or liver cancer.Most of hepatitis C can be cured.Effective treatment measures include pegylated long-acting interferon and direct-acting antivirals(DAAs).However,previous studies have found that patients can develop depression during interferon treatment,and the related mechanisms may be related to abnormal activity of hypothalamic pituitary adrenal(HPA)axis,brain-derived neurotrophic factor hypothesis,polyunsaturated fatty acid(PUFAs)disorder,inflammation theory,single nucleotide polymorphism of specific genes(SNP)and cytokine theory;in addition,depression may also occur in DAAs treatment.This paper reviews the research progress on the pathogenesis of depression related to interferon and DAAs in the treatment of chronic hepatitis C.
