痰中IL-25、HNP1-3、YKL-40水平与患儿哮喘严重程度及气道重塑的相关性分析

摘要 目的探讨痰中白细胞介素-25(IL-25)、人中性粒细胞防御素(HNP1-3)、人软骨糖蛋白-39(YKL-40)水平与患儿哮喘严重程度及气道重塑的相关性。方法选取2019年1月至2020年6月在海安市人民医院就诊的214例急性哮喘患儿作为研究对象,根据《2014年全球哮喘防治创议委员会儿童支气管哮喘最新修订指南》,将所有患儿分为轻度组、中度组、重度组,分别为58例、91例和65例,同期选取50例健康幼儿设为对照组;对各组的肺功能指标、痰中IL-25、HNP1-3、YKL-40水平以及上皮基底膜厚度进行检测和比较。结果轻度组、中度组、重度组第1秒用力呼气量(FEV_(1))、用力肺活量(FVC)、最大呼气峰流速(PEF)水平、基底膜厚度显著低于对照组,而IL-25、HNP1-3、YKL-40水平高于对照组,且随着病情的加重,FEV_(1)、FVC、PEF水平显著降低,IL-25、HNP1-3、YKL-40水平及基底膜厚度显著增加(P<0.05);经Pearson检验,痰中IL-25、HNP1-3、YKL-40水平与患儿哮喘严重程度及气道重塑呈正相关。结论哮喘患儿痰中IL-25、HNP1-3、YKL-40水平与哮喘严重程度及气道重塑呈正相关,其检测水平可反映患儿的病情严重程度。 Objective To investigate the correlation of sputum interleukin-25(IL-25),human neutrophil defensin(HNP1-3)and chitosanase-like protein YKL-40 between asthma severity and airway remodeling in children.Methods 214 children with acute asthma who received medical care in the hospital from January to June 2020 were recruited as study subjects and were divided into the mild,moderate and severe groups,respectively consisting of 58,91and 65 patients.Another 50 healthy children were selected within the same time span as the control group.Detections and comparisons were made concerning lung function indexes,sputum IL-25,HNP1-3,YKL-40 levels and epithelial basement membrane thickness in the 4 groups.Results FEV_(1),FVC,PEF and basement membrane thickness in the mild,moderate and severe groups were all lower than those in the control group,while the levels of IL-25,HNP1-3 and YKL-40 were higher than those in the control group.With the aggravation of the disease,the levels of FEV_(1),FVC and PEF decreased significantly,and the levels of IL-25,HNP1-3,YKL-40 and basement membrane thickness increased significantly(P<0.05).Pearson test showed that the levels of IL-25,HNP1-3 and YKL-40 in sputum were positively correlated with the severity of asthma and airway remodeling.Conclusion The levels of IL-25,HNP1-3 and YKL-40 in the sputum of asthmatic children were positively correlated with the severity of asthma and airway remodeling.
