2020年某三甲医院24000张门急诊处方点评与分析

Rview and Analysis of 24000 Outpatient and Emergency Prescriptions in a Tertiary Hospital in 2020

目的调查分析某三甲医院2020年门急诊处方,旨在了解门急诊用药情况,提高医院用药合理性,确保患者用药安全。方法随机抽取该院2020年1-12月门急诊西药处方24000张(每月抽取2000张),以《处方管理办法》、《医院处方点评管理规范(试行)》等相关法律法规为依据,对抽取处方进行点评和分析。将不合格处方分为不规范处方、用药不适宜处方及超常处方,并采用帕累托图分析不合格处方的原因。结果24000张处方中,合格处方23652张,合格率98.55%,不合格处方数348张,不合格处方占抽查处方的1.45%,不合格处方中包括不规范处方27张(7.76%),不适宜处方168张(48.28%),超常处方153张(43.96%);所抽查的处方中包含有抗菌药物处方8280张,抗菌药物使用率34.50%。结论该院门急诊处方合格率较高,但仍存在一些不合理用药之处,需医师和药师共同努力,加强学习和培训,促进门急诊合理用药。

Objective To investigate and analyze the prescriptions of outpatient and emergency departments in a tertiary hospital in 2020 in order to understand the situation of drug use in outpatient and emergency departments and improve the rationality use of drug in hospital to ensure the safety of patients.Methods 24000 prescriptions of western medicine in outpatient and emergency departments of the hospital from January to December 2020 were randomly selected(2000 prescriptions per month).The selected prescriptions were evaluated and analyzed on the basis of Prescription Management Measures,Hospital Prescription Evaluation Management Standard(Trial)and other relevant laws and regulations.Unreasonable prescriptions were regarded as non-standard prescriptions,inappropriate prescriptions and supernormal prescriptions.Pareto chart was used to analyze the reasons for unqualified prescriptions.Results Among the 24,000 prescriptions,the number of qualified prescriptions was 23,652,and the qualified rate was 98.55%.The total number of unqualified prescriptions was 348,and the unqualified prescriptions accounted for 1.45%.The unqualified prescriptions included 27 irregular prescriptions(7.76%),168 inappropriate prescriptions(48.28%),and 153 abnormal prescriptions(43.96%).In addition,8280 prescriptions were for antibiotics,and the usage rate of antibiotics was 34.50%.Conclusion The qualified rate of outpatient and emergency prescriptions in this hospital was relatively high,but it existed some unreasonable use of drug.It is necessary for doctors and pharmacists to make efforts to strengthen study and training to promote rational use of drug.
