预见护理和综合性护理对直肠癌造瘘患者术后自护能力的干预价值

Intervention Value of Predictive Nursing and Comprehensive Nursing for Postoperative Self-care of Rectal Cancer Patients with Fistula

目的探讨对直肠癌造瘘患者采用预见性护理+综合性护理方式进行干预后对其术后自护能力的影响。方法采取对比研究的方式,选取我院2018年6月至2020年4月收治的80例直肠癌造瘘患者,采用数字奇偶法分组,护理组(40例)采用基础护理+预见护理+综合性护理展开造瘘护理干预,常规组(40例)采用基础护理展开造瘘护理干预,就组间癌因性疲乏比例以及术后自护能力评分展开对比。结果护理组直肠癌造瘘患者无疲乏比例(15.00%)、轻度疲乏比例(70.00%)均高于常规组(0.00%、17.50%),差异有统计学意义(P<0.05);中度疲乏比例(10.00%)、重度疲乏比例(5.00%)均低于常规组(50.00%、32.50%),差异有统计学意义(P<0.05);护理组直肠癌造瘘患者自我护理技能评分(36.19±6.43)分、自我护理责任感评分(25.31±1.41)分、自我概念评分(19.21±5.13)分、健康知识水平评分(59.05±6.45)分以及ESCA总得分(141.55±20.21)分均高于常规组的(29.61±6.47)分、(23.08±1.19)分、(15.49±5.42)分、(51.16±5.62)分、(112.95±22.70)分,差异有统计学意义(P<0.05)。结论预见护理+综合性护理方法的有效运用可使直肠癌造瘘患者癌因性疲乏显著减轻,同时提升其自护能力,最终实现直肠癌造瘘患者的有效预后。

Objective To explore the influence of predictive nursing+comprehensive nursing on postoperative self-protection ability of rectal cancer patients with colostomy.Methods A total of 80 rectal cancer patients with colostomy admitted to our hospital from June 2018 to April 2020 were grouped by odd-even method.Nursing group(40 cases):basic nursing+predictive nursing+comprehensive nursing to carry out fistula nursing intervention.Conventional group(40 cases):basic nursing to carry out fistula nursing intervention.The ratio of cancer-related fatigue and postoperative self-care ability were compared between groups.Results The proportion of fatigue free(15.00%)and mild fatigue(70.00%)in the nursing group were significantly higher than that in the conventional group(0.00%,17.50%),the differences were statistically significant(P<0.05).The proportion of moderate fatigue(10.00%)and severe fatigue(5.00%)was significantly lower than that of conventional group(50.00%,32.50%),the differences were statistically significant(P<0.05).In the nursing group,the scores of self-care skills,sense of self-care,self-concept,health knowledge and ESCA were(36.19±6.43),(25.31±1.41),(19.21±5.13),(59.05±6.45)and(141.55±20.21)points,significantly higher than the conventional group(29.61±6.47),(23.08±1.19),(15.49±5.42),(51.16±5.62),(112.95±22.70)points,the differences were statistically significant(P<0.05).Conclusion The effective use of predictive nursing+comprehensive nursing can significantly reduce cancer-induced fatigue in rectal cancer patients with colostomy,and at the same time significantly improve the ability of self-care,and ultimately achieve effective prognosis in rectal cancer patients with colostomy.
