腹部超声联合阴道超声诊断在妇产科急腹症中的应用有效性评价

Evaluation of the effectiveness of abdominal ultrasound combined with vaginal ultrasound in the diagnosis of acute abdomen in obstetrics and gynecology

目的:探讨针对妇产科收治的急腹症患者运用腹部超声与阴道超声联合诊断的方案有效性。方法:选取2019年6月—2021年6月德州市德城区妇女儿童医院收治的疑似急腹症患者529例,分别采用腹部超声和阴道超声技术实施检查,并以病理检查结果为金标准,就腹部超声和阴道超声技术单用、联用诊断效能展开对比。结果:经腹部超声诊断急腹症的灵敏度为81.71%,特异度为98.16%,准确率为90.17%;经阴道超声诊断灵敏度为82.49%,特异度为98.16%,准确率为90.55%;经腹部联合经阴道超声诊断灵敏度为96.89%,特异度为99.63%,准确率为98.30%。采用联合超声检查在诊断灵敏度以及准确率方面均高于经腹、经阴道超声单用,差异具有统计学意义(P<0.05)。结论:针对临床收治的疑似妇产科急腹症患者,运用腹部超声联合阴道超声诊断技术方案实施诊断,在诊断效能上明显高于上述两种超声技术单用,价值十分突出,值得临床引起重视,并积极推广运用。

Objective To explore the effectiveness of abdominal ultrasound combined with vaginal ultrasound in diagnosis of acute abdomen in obstetrics and gynecology.Methods A total of 529 patients with suspected acute abdomen admitted to Dezhou Decheng District Women’s and Children’s Hospital from June 2019 to June 2021 were selected and examined by abdominal ultrasound and vaginal ultrasound respectively.The diagnostic efficacy of abdominal ultrasound and vaginal ultrasound alone or in combination was compared using pathological examination result as the golden standard.Results The sensitivity,specificity and accuracy of transabdominal ultrasound in the diagnosis of acute abdomen were 81.71%,98.16% and 90.17% respectively.The sensitivity,specificity and accuracy of transvaginal ultrasound were 82.49%,98.16% and 90.55% respectively.The sensitivity,specificity and accuracy of transabdominal combined with transvaginal ultrasound were 96.89%,99.63% and 98.30%,respectively.The diagnostic sensitivity and accuracy of combined ultrasonography were higher than that of transabdominal and transvaginal ultrasonography alone,with statistical significance(P <0.05).Conclusion For the suspected patients with acute abdomen in obstetrics and gynecology admitted to the clinic,the application of abdominal ultrasonic diagnosis combined with vaginal ultrasonic diagnosis technology has significantly higher diagnostic efficiency than the above two ultrasound technologies alone,which is of outstanding value and worthy of clinical attention and active promotion and application.
