调营和卫益气养血法治疗血虚型糖尿病性皮肤瘙痒症的临床应用

Clinical Application of Harmonizing Yingfen and Weifen and Reinforcing Qi and Nourishing Blood Method in the Treatment of Blood Defi ciency Syndrome Diabetic Skin Pruritus

目的:探讨调营和卫益气养血法治疗血虚型糖尿病性皮肤瘙痒症的有效性及安全性。方法:按照随机数字表法将2019年1月-12月笔者医院收治的120例血虚型糖尿病性皮肤瘙痒症患者分为对照组与观察组,每组60例。对照组常规治疗,观察组在常规治疗基础上加用调营和卫益气养血的中药组方,均连续治疗4周。比较两组的临床疗效,病情指标评分(瘙痒程度、瘙痒发生频率、瘙痒持续时间、继发皮损),瘙痒相关细胞因子(SCF、DYN、IgE),血糖(FBG、P2hPG、HbA1c),血脂(TC、TG、HDL-C、LDL-C),血压(SBP、DBP)及不良反应情况。结果:观察组的总有效率[88.33%(53/60)vs 53.33%(32/60)]明显高于对照组(P<0.05)。治疗后,观察组的瘙痒程度、瘙痒发生频率、瘙痒持续时间及继发皮损评分均明显低于对照组(P<0.05)。治疗后,两组的TC、TG、HDL-C、LDL-C、SBP、DBP无明显变化(P>0.05),血清SCF、DYN、IgE水平及FBG、P2hPG、HbA1c均明显降低(P<0.05),观察组的血清SCF、DYN、IgE水平及FBG、P2hPG、HbA1c均明显低于对照组(P<0.05)。两组的不良反应发生率比较差异无统计学意义(P>0.05)。结论:调营和卫益气养血法治疗血虚型糖尿病性皮肤瘙痒症的疗效显著,安全性高,血糖得到更好控制。

Objective To discuss the effectiveness and safety of harmonizing yingfen and weifen and reinforcing qi and nourishing blood method in the treatment of blood deficiency syndrome diabetic skin pruritus.Methods According to the random number method,120 patients with blood defi ciency syndrome diabetic skin pruritus who were randomly divided into control group and observation group in author hospital from January to December 2019,with 60 patients in each group.Patients were treated with conventional therapy in the control group,while patients were added harmonizing yingfen and weifen and reinforcing qi and nourishing blood method based on conventional therapy in the observation group,all the patients were treated continuously for 4 weeks.The clinical effi cacy,disease state index score(itch degree,itch frequency,itch duration,secondary skin lesions),pruritus-related cytokines(SCF,DYN,IgE),blood glucose(FBG,P2hPG,HbA1c),blood pressure(SBP,DBP)and adverse reactions were compared between the two groups.Results The total eff ective rate[88.33%(53/60)vs 53.33%(32/60)]in observation group was signifi cantly higher than that in control group(P<0.05).The itch degree,itch frequency,itch duration and secondary skin lesions scores in in observation group were signifi cantly lower than that in control group(P<0.05).After treatment,there were no signifi cant changes in TC,TG,HDL-C,LDL-C,SBP and DBP in the two groups(P>0.05),serum levels of SCF,DYN,IgE and FBG,P2hPG and HbA1c was signifi cantly reduced(P<0.05),and serum levels of SCF,DYN,IgE and FBG,P2hPG,HbA1c in the observation group was signifi cantly lower than those in the control group(P<0.05).There was no signifi cant diff erence in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The curative eff ect was signifi cant and safety was high of harmonizing yingfen and weifen and reinforcing qi and nourishing blood method in the treatment of blood defi ciency syndrome diabetic skin pruritus.
