扶正抗癌Ⅱ方联合自血穴注治疗晚期肺癌癌因性疲乏的临床观察

Clinical Observation of Fuzheng Kang’aiⅡPrescription Combined with Self-blood Acupoint Injection in the Treatment of Advanced Lung Cancer Cancer-related Fatigue

目的:研究扶正抗癌Ⅱ方联合自血穴注治疗晚期肺癌癌因性疲乏的临床效果。方法:选取2018年1月-2020年1月于广东省中西医结合医院肿瘤科门诊及住院部治疗的晚期肺癌癌因性疲乏患者90例,按随机数字表法分成联合治疗组(30例,给予扶正抗癌Ⅱ方联合自血穴注治疗)、扶正抗癌组(30例,给予扶正抗癌Ⅱ方治疗)和空白对照组(30例,给予基础支持治疗及对症处理)。各组均治疗8周。对比治疗前后各组的疲劳程度(采用Piper疲乏自评量表评定)、生活质量[采用欧洲癌症治疗研究组织生命质量测定量表(QLQ-C30)评价]、白介素-6(IL-6)水平、T细胞亚群、血清皮质醇水平、中医症候积分。结果:治疗后,联合治疗组、扶正抗癌组的疲劳程度评分及中医症候积分均明显低于空白对照组,且联合治疗组低于扶正抗癌组,差异均有统计学意义(P<0.05)。治疗后,联合治疗组、扶正抗癌组的生活质量各项评分明显优于空白对照组,且联合治疗组优于扶正抗癌组,差异均有统计学意义(P<0.05)。治疗后,联合治疗组、扶正抗癌组的IL-6及皮质醇水平明显低于空白对照组,且联合治疗组低于扶正抗癌组,差异均有统计学意义(P<0.05)。治疗后,联合治疗组及扶正抗癌组的CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平明显高于空白对照组,且联合治疗组高于扶正抗癌组,而CD8^(+)明显低于空白对照组,且联合治疗组低于扶正抗癌组,差异均有统计学意义(P<0.05)。结论:扶正抗癌Ⅱ方联合自血穴注治疗晚期肺癌癌因性疲乏的临床效果较好,还可改善患者的生活质量及免疫功能,值得推广。

Objective:To study the clinical effect of Fuzheng Kang’aiⅡPrescription combined with self-blood acupoint injection in the treatment of advanced lung cancer cancer-induced fatigue.Method:A total of 90 advanced lung cancer patients with cancerrelated fatigue treated in the outpatient department and inpatient department of Guangdong Hospital of Integrated Traditional Chinese and Western Medicine from January 2018 to January 2020 were selected,and they were divided into the combined treatment group(30 cases,Fuzheng Kang’aiⅡPrescription combined with self-blood acupoint injection),Fuzheng Kang’ai group(30 cases,Fuzheng Kang’aiⅡPrescription)and blank control group(30 cases,basic support treatment and symptomatic treatment)according to the random number table method.All groups were treated for 8 weeks.Before and after treatment,the fatigue severity(assessed by Piper fatigue self-rating scale),quality of life[assessed by the European Organization for Cancer Research quality of life scale(QLQ-C30)],interleukin-6(IL-6)level,T cell subsets,serum cortisol level and traditional Chinese medicine(TCM)symptom score were compared among the groups.Result:The fatigue scores and TCM syndrome scores of the combined treatment group and the Fuzheng Kang’ai group were significantly lower than those of the blank control group,and the combined treatment group was lower than that in the Fuzheng Kang’ai group,with statistical significances(P<0.05).The scores of quality of life after treatment in the combined treatment group and the Fuzheng Kang’ai group were significantly better than those in the blank control group,and the combined treatment group was better than that in the Fuzheng Kang’ai group,with statistical significance(P<0.05).After treatment,the IL-6 and cortisol levels in the combined treatment group and the Fuzheng Kang’ai group were significantly lower than the blank control group,and those in the combined treatment group were lower than those in the Fuzheng Kang’ai group,with statistical significances(P<0.05).After treatment,the CD3^(+),CD4^(+),CD4^(+)/CD8^(+)levels in the combination treatment group and the Fuzheng Kang’ai group were significantly higher than those in the blank control group,and the combined treatment group were lower than those in the Fuzheng Kang’ai group,while the CD8^(+)level in the combination treatment group and the Fuzheng Kang’ai group was significantly lower than that in the blank control group,and the combination treatment group was lower than that of the Fuzheng Kang’ai group,with statistical significance(P<0.05).Conclusion:Fuzheng Kang’aiⅡPrescription combined with self-blood acupoint injection has a good clinical effect on advanced lung cancer patients with cancer-related fatigue,and can improve the quality of life and immune function of patients,which is worth popularizing.
