翼状胬肉切除联合不同移植术治疗对翼状胬肉患者的影响

Influence of Pterygium Excision Combined with Different Transplantation Surgery on Patients with Pterygium

目的:观察翼状胬肉切除联合不同移植术治疗对翼状胬肉患者的影响。方法:选取四会市中医院2019年6月-2021年10月眼科收治翼状胬肉切除手术患者90例。按照随机数字法分成A组、B组、C组,每组30例。A组联合自体角膜缘干细胞移植术治疗,B组联合自体结膜瓣移植术治疗,C组联合羊膜移植术治疗。比较三组泪膜破裂时间(BUT)、泪液分泌试验(SIT)、不适症状分级、复发情况及伤口恢复时间。结果:三组术后BUT均较术前改善;B组、C组高于A组(P<0.05)。三组术后SIT均较术前改善;B组、C组高于A组(P<0.05)。A组、B组术后复发率均低于C组,伤口修复时间短于C组(P<0.05)。结论:翼状胬肉患者以翼状胬肉切除手术联合自体结膜瓣移植术或联合羊膜移植术治疗,BUT及SIT均优于联合自体角膜缘干细胞移植术治疗,但联合自体角膜缘干细胞移植术或自体结膜瓣移植术复发率低、伤口修复时间短。

Objective:To observe the effect of pterygium excision combined with different transplantation on ocular surface repair and functional recovery in patients with pterygium.Method:A total of 90 patients who underwent pterygium excision in Sihui Hospital of Traditional Chinese Medicine from June 2019 to October 2021 were selected,and divided into group A,group B and group C by random number method,with 30 cases in each group.Group A combined with autologous limbal stem cell transplantation,group B combined with autologous conjunctival flap transplantation,and group C combined with amniotic membrane transplantation.The break-up time of tear film(BUT),Schirmer test(SIT),classification of discomfort symptoms,recurrence and wound recovery time were compared among the three groups.Result:The postoperative BUT in the three groups were better than those before operation,those in the group B and C were higher than those in the group A(P<0.05).The postoperative SIT in the three groups were better than those before operation,the SIT of group B and group C were higher than those of group A(P<0.05).The postoperative recurrence rate in the group A and group B were lower than those in the group C,and the wound repair time was shorter than that in group C(P<0.05).Conclusion:The BUT and SIT of patients with pterygium treated with pterygium excision combined with autologous conjunctival flap transplantation or amniotic membrane transplantation are better than those treated with autologous limbal stem cell transplantation,but the recurrence rate and wound repair time of patients treated with autologous limbal stem cell transplantation and autologous conjunctival flap transplantation are low and short.
