腹腔镜手术与开腹手术治疗异位妊娠手术切口不使用抗菌药临床效果及预后分析

Clinical effect and prognosis analysis of laparoscopic surgery and open surgery for ectopic pregnancy without the use of antimicrobials

目的分析异位妊娠治疗中开腹手术与腹腔镜手术后手术切口不使用抗菌药的临床效果及预后。方法100例异位妊娠患者,随机分为腹腔镜手术组和开腹手术组,每组50例。腹腔镜手术组采用腹腔镜手术,开腹手术组采用开腹手术,两组手术切口均不使用抗菌药。比较两组患者手术相关指标及术后镇痛情况、并发症(切口感染、发热)发生情况。结果腹腔镜手术组患者术中出血量(47.41±7.52)ml少于开腹手术组的(82.61±9.08)ml,术后肛门排气时间(9.15±1.46)h、术后腹痛消失时间(39.67±3.48)h、术后住院时间(5.49±1.00)d均短于开腹手术组的(19.40±3.25)h、(62.71±6.20)h、(7.60±1.61)d,差异均具有统计学意义(P<0.05);但两组手术时间比较,差异无统计学意义(P>0.05)。腹腔镜手术组患者的术后镇痛率8.00%(4/50)低于开腹手术组的42.00%(21/50),术后并发症发生率4.00%(2/50)低于开腹手术组的16.00%(8/50),差异均具有统计学意义(P<0.05)。结论异位妊娠治疗中腹腔镜手术后手术切口不使用抗菌药临床效果及预后好。

Objective To analyze the clinical effect and prognosis of laparoscopic surgery and open surgery for ectopic pregnancy without the use of antimicrobials.Methods A total of 100 patients with ectopic pregnancy were randomly divided into laparoscopic surgery group and open surgery group,with 50 cases in each group.The laparoscopic surgery group used laparoscopic surgery,and the open surgery group used open surgery.No antimicrobials were used for the surgical incision in both groups.The operation-related indicators,postoperative analgesia,and complications(incision infection,fever)were compared between the two groups.Results The intraoperative blood loss(47.41±7.52)ml in the laparoscopic surgery group was less than(82.61±9.08)ml in the open surgery group,and the postoperative anal exhaust time(9.15±1.46)h,postoperative abdominal pain disappearance time(39.67±3.48)h and postoperative hospital stay(5.49±1.00)d were shorter than(19.40±3.25)h,(62.71±6.20)h,and(7.60±1.61)d in the open surgery group.All the differences were statistically significant(P<0.05).But there was no statistically significant difference in operation time between the two groups(P>0.05).The postoperative analgesia rate 8.00%(4/50)of the laparoscopic surgery group was lower than 42.00%(21/50)of the open surgery group,and the postoperative complication rate 4.00%(2/50)was lower than 16.00%(8/50)of the open surgery group.All the differences were statistically significant(P<0.05).Conclusion Laparoscopic surgery for ectopic pregnancy without the use of antimicrobials in the surgical incision has a good clinical effect and prognosis.
