牙冠延长术治疗牙体劈裂的疗效分析

Analysis of the efficacy of crown lengthening in the treatment of tooth fracture

目的分析牙冠延长术治疗牙体劈裂的疗效。方法68例(68颗)牙体劈裂患者,采用随机数字表法分为对照组和观察组,每组34例。对照组接受牙龈切除术治疗,观察组接受牙冠延长术治疗。比较两组患者的各项牙周指数[菌斑指数(PLI)、龈沟出血指数(SBI)、牙周探诊深度(PD)]、牙齿松动发生情况、牙体修复效果。结果两组患者术前、术后3个月、术后6个月的PLI、SBI、PD组间对比差异无统计学意义(P>0.05);术后3、6个月,两组患者的PLI、SBI、PD均明显低于本组术前,差异有统计学意义(P<0.05)。观察组患者牙齿松动发生率0低于对照组的17.65%,差异有统计学意义(χ^(2)=6.581,P=0.010<0.05)。观察组患者牙体修复总有效率为100.00%,显著高于对照组的88.24%,差异有统计学意义(χ^(2)=4.250,P=0.039<0.05)。结论牙冠延长术用于治疗牙体劈裂患者可有效修复患者的牙周组织,有利于改善患者的牙周指数,减少牙齿松动,牙体修复效果及综合应用价值明显优于传统牙龈切除术,值得推荐。

Objective To analyze the efficacy of crown lengthening in the treatment of tooth fracture.Methods A total of 68 patients(68 teeth)with tooth fracture were divided into control group and observation group by random numerical table,with 34 cases in each group.The control group received gingivectomy,and the observation group received crown lengthening.The periodontal indices[plaque index(PLI),sulcus bleeding index(SBI),periodontal probing depth(PD)],occurrence of tooth loosening and dental restoration effect were compared between the two groups.Results There was no statistically significant difference in PLI,SBI and PD between the two groups before surgery,3 months after surgery,and 6 months after surgery(P>0.05).At 3 and 6 months after surgery,the PLI,SBI and PD of the two groups were significantly lower than those before surgery,and the difference was statistically significant(P<0.05).The incidence of tooth loosening 0 in the observation group was lower than 17.65%in the control group,and the difference was statistically significant(χ^(2)=6.581,P=0.010<0.05).The total effective rate of tooth restoration in the observation group was 100.00%,which was significantly higher than 88.24%in the control group,and the difference was statistically significant(χ^(2)=4.250,P=0.039<0.05).Conclusion Crown lengthening for the treatment of patients with tooth fracture can effectively repair the patient’s periodontal tissue,which is conducive to improving the patient’s periodontal index and reducing tooth loosening,and the restorative effect and comprehensive application value of the dentition is significantly better than that of traditional gingivectomy,which is recommended.
