基于RIGHT神经根型颈椎病临床实践指南的报告质量研究

Research on Reporting Quality based on RIGHT Clinical Practice Guidelines for Cervical Radiculopathy

目的:基于RIGHT清单评价国内外关于神经根型颈椎病的临床实践指南的报告质量,分析报告内容中的不足之处,进一步规范神经根型颈椎病指南的报告。方法:检索中英文数据库中以及指南相关网站上关于神经根型颈椎病的临床实践指南,检索时限均为2007年1月1日至2020年12月31日。由2名研究人员独立筛选文献并交叉核对,采用RIGHT对关于神经根型颈椎病的指南进行报告质量研究。结果:最终纳入13篇指南,其中8篇为中文指南,5篇为英文指南;中医指南为2篇,西医指南为11篇。总体平均报告率为52.97%,其中领域一基本信息方面(80.77%)、领域二背景(66.35%)和领域三证据(60%)的报告率均大于50%,领域四推荐意见(43.96%)、领域五评审和质量保证(30.77%)、领域六资金资助与利益冲突声明和管理(38%)和领域七其他方面(20.51%)的报告率低于50%。国内发表的指南平均报告率(47.94%)低于国外(57.86%),其中国内指南最高的报告率为74.28%,国外指南最高的报告率为82.85%。结论:国内外神经根型颈椎病指南的报告质量有待提高。指南制定者可以结合RIGHT清单规范撰写流程,充分报告指南关键信息和内容,提升神经根型颈椎病指南报告的完整性、透明性,便于指南推荐意见的推广和实施。

Objective:Based on the RIGHT checklist,to evaluate the report quality of clinical practice guidelines on cervical radiculopathy at home and abroad,analyze the deficiencies in the report content,and further standardize the reports of the guidelines for cervical radiculopathy.Methods:The clinical practice guidelines for cervical radiculopathy in Chinese and English databases and guideline-related websites were searched from January 1,2007 to December 31,2020.Two researchers independently screened and cross-checked the literature,using RIGHT for reporting quality studies for guidelines on cervical radiculopathy.Results:Thirteen guidelines were finally included,including 8 Chinese guidelines and 5 English guidelines;2 Chinese medicine guidelines and 11 Western medicine guidelines.The overall average reporting rate was 52.97%,of which the reporting rates of basic information in Domain 1(80.77%),background in Domain 2(66.35%)and evidence in Domain 3(60%)were all greater than 50%,and recommendations in Domain 4(43.96%),Domain 5 Review and Quality Assurance(30.77%),Domain 6 Funding and Conflict of Interest Declaration and Management(38%)and Domain 7 Others(20.51%)reported less than 50%.The average reporting rate of domestic published guidelines(47.94%)was lower than that of foreign countries(57.86%),of which the highest reporting rate of domestic guidelines was 74.28%,and the highest reporting rate of foreign guidelines was 82.85%.Conclusion:The reporting quality of the guidelines for cervical radiculopathy at home and abroad needs to be improved.Guideline makers can standardize the writing process in combination with the RIGHT list,fully report the key information and content of the guideline,improve the completeness and transparency of the guideline report for cervical radiculopathy,and facilitate the promotion and implementation of the guideline recommendations.
