门诊西药房药品管理和发放差错分析

Error Analysis of Drug Management and Distribution in Outpatient Western Pharmacy

目的分析研究门诊西药房药品管理和发放差错。方法选取2017年1月-2021年1月于该院就诊并开具处方取药的患者50例,回顾性分析患者的临床资料,分析药品发放中出现错误的事件类型及药品发放中出现错误的原因。结果药品剂型发放错误占比32.00%、药品品类错误占比48.00%、药品剂量发放错误占比14.00%、药品时效错误占比6.00%。药品摆放位置错误占比26.00%、药品管理不足占比14.00%、药剂师观念薄弱占比18.00%、单人配发占比42.00%。结论相关医务人员应该重视药品发放的问题,找出问题后实施针对性的改进措施,持续提高门诊西药房的工作质量,很大程度上提高医院的药品管理水平。

Objective To analyze and study drug management and distribution errors in outpatient western pharmacies.Methods 50 patients who visited the hospital and received prescriptions to take medicines January 2017 to January 2021 were selected.The clinical data of the patients were retrospectively analyzed,and the types of events that occurred in drug dispensing and the reasons for errors in drug dispensing were analyzed.Results Drug dosage form distribution errors accounted for 32.00%,drug category errors accounted for 48.00%,drug dosage errors accounted for 14.00%,and drug aging errors accounted for 6.00%.Drug wrong placement accounted for 26.00%,insufficient drug management accounted for 14.00%,pharmacists'weak concept accounted for 18.00%,and single-person dispensing accounted for 42.00%.Conclusion Relevant medical staff should pay attention to the problem of drug distribution,implement targeted improvement measures after identifying the problem,continue to improve the work quality of outpatient western pharmacies,and greatly improve the hospital's drug management level.
