经阴道Er:YAG激光联合电针治疗女性压力性尿失禁与盆腔器官脱垂 被引量:1

Transvaginal Er:YAG Laser Combined with Electroacupuncture in the Treatment of Female Stress Urinary Incontinence and Pelvic Organ Prolapse

摘要 目的:探究女性压力性尿失禁(stress urinary incontinence in women,SUI)与盆腔器官脱垂(pelvic organ prolapse,POP)采用经阴道Er:YAG激光联合电针治疗的临床效果。方法:回顾性分析2019年1月-2020年5月在东莞市南城医院就诊的单纯SUI、单纯POP、SUI合并POP患者共180例的临床资料,分别采用Er:YAG激光与Er:YAG激光联合电针治疗,根据治疗方法差异分组,单纯SUI激光治疗为A_(1)组(n=40),单纯SUI联合治疗为A_(2)组(n=40),单纯POP激光治疗为B_(1)组(n=20),单纯POP联合治疗为B_(2)组(n=20),SUI合并POP激光治疗为C_(1)组(n=30),SUI合并POP联合治疗为C_(2)组(n=30),对比疗效。结果:A_(2)组总有效率为95.00%,B_(2)组为95.00%,C_(2)组为90.00%,分别高于A_(1)组的80.00%、B_(1)组的65.00%、C_(1)组的70.00%(P<0.05)。A_(2)、B_(2)、C_(2)组各评估指标分别优于A_(1)、B_(1)、C_(1)组(P<0.05)。A_(2)、B_(2)、C_(2)组阴道收缩肌电值、阴道动态压力高于A_(1)、B_(1)、C_(1)组,且阴道收缩持续时间均明显长于A_(1)、B_(1)、C_(1)组(P<0.05),结论:经阴道Er:YAG激光联合电针可增强女性SUI和POP疾病治疗效果,进而改善患者生活质量,具有较好的应用价值,值得推广。 Objective:To explore the clinical effect of transvaginal Er:YAG laser combined with electroacupuncture for female stress urinary incontinence(SUI)and pelvic organ prolapse(POP).Method:A retrospective analysis was performed on the clinical data of 180 patients with simple SUI,simple POP and SUI combined POP treated in Dongguan Nancheng Hospital from January 2019 to May 2020,Er:YAG laser and Er:YAG laser combined with electroacupuncture,respectively.According to the differences treatment methods,SUI laser treatment alone was as group A_(1)(n=40),SUI combined treatment alone was as group A_(2)(n=40),POP laser treatment alone was as group B_(1)(n=20),POP combined treatment alone was as group B_(2)(n=20),SUI combined treatment with POP laser was as group C_(1)(n=30),SUI combined treatment combined treatment with POP was as group C_(2)(n=30),and the efficacy among the groups was compared.Result:The total effective rate was 95.00%in group A_(2),95.00%in group B_(2)and 90.00%in group C_(2),which were higher than 80.00%in group A_(1),65.00%in group B_(1)and 70.00%in group C_(1)(P<0.05).The evaluation indexes in A_(2),B_(2)and C_(2)groups were better than those in group A_(1),B_(1)and C_(1)(P<0.05).The electromyographic value and dynamic vaginal pressure of vaginal contractions in group A_(2),B_(2)and C_(2)were higher than those in group A_(1),B_(1)and C_(1),and the duration of vaginal contractions were significantly longer than those in group A_(1),B_(1)and C_(1)(P<0.05).Conclusion:Transvaginal Er:YAG laser combined with electroacupuncture can enhance the treatment effect of female SUI and POP disease,and then improve the quality of life of patients,which has strong application value and is worthy of further research and promotion.
