中医护理技术对痛风性关节炎患者干预效果的meta分析

A meta-analysis of the intervention effect of traditional Chinese medicine nursing techniques on patients with gouty arthritis

目的系统评价中医护理技术对痛风性关节炎患者的干预效果。方法计算机检索PubMed、Embase、CINAHL、MedLine、The Cochrane Library、Web of Science、中国知网、CBM、万方和维普数据库,找出中医护理技术应用于痛风性关节炎患者的随机对照试验(RCT),由2位研究者根据纳入排除标准进行筛选及质量评价,采用RevMan5.3软件进行meta分析。结果最终纳入12篇文章,共1005例患者。与常规护理组比较,中医护理联合常规护理组患者的疼痛视觉模拟评分降低[均数差(MD)=-1.86,95%置信区间(95%CI)为-2.58~-1.14,P<0.05],关节直径减小(MD=-0.76,95%CI为-0.85~-0.68,P<0.05),血尿酸水平下降(MD=-67.06,95%CI为-67.94~-66.18,P<0.05),疗效评价提高(比值比=4.15,95%CI为2.66~6.50,P<0.05)。结论在常规护理基础上运用中医护理技术能减轻痛风性关节炎患者的关节疼痛、肿胀,降低血尿酸水平,提高护理疗效。

Objective To systematically review the intervention effect of traditional Chinese medicine nursing techniques on patients with gouty arthritis.Methods PubMed,Embase,CINAHL,MedLine,The Cochrane Library,Web of Science,CNKI,CBM,Wanfang and VIP databases were searched by computer to find out randomized controlled trials(RCTs)of traditional Chinese medicine nursing techniques applied to patients with gouty arthritis.Screening and quality evaluation were performed by 2 investigators according to the inclusion and exclusion criteria,and meta-analysis was performed using RevMan5.3 software.Results Finally,12 articles with 1005 patients were included.Compared with the routine nursing group,the traditional Chinese medicine nursing combined with routine nursing group had lower pain visual analog score(MD=-1.86,95%CI=-2.58 to-1.14,P<0.05),joint diameter decreased(MD=-0.76,95%CI-0.85 to-0.68,P<0.05),blood uric acid level decreased(MD=-67.06,95%CI-67.94 to-66.18,P<0.05),and efficacy evaluation improved(OR=4.15,95%CI 2.66-6.50,P<0.05).Conclusion Traditional Chinese medicine nursing techniques combined with routine nursing can reduce joint pain and swelling in patients with gouty arthritis,reduce blood uric acid levels,and improve nursing efficacy.
