膝关节内侧间室骨性关节炎患者采用全膝关节置换术的效果研究

The Effect of Total Knee Arthroplasty in Patients with Medial Compartment Osteoarthritis of the Knee Joint

目的探究全膝关节置换术在膝关节内侧间室骨性关节炎患者治疗中的应用价值。方法研究纳入该院2016年10月—2021年11月收治的100例膝关节内侧间室骨性关节炎患者,经双盲法随机分为对照组与观察组,各50例,分别实施关节镜手术、全膝关节置换术。对两组治疗效果展开比较。结果观察组手术用时、术中出血量、住院时间及首次直腿抬高时间均显著优于对照组,差异有统计学意义(t=9.563、6.020、4.126、6.726,P<0.001)。术后,观察组C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平均显著低于对照组,差异有统计学意义(t=9.643、14.412、5.246,P<0.001);相较于对照组,观察组术后WHOQOL-100评分明显更高、PSQI评分显著更低,差异有统计学意义(t=4.649、8.060,P<0.001);观察组KSS评分、VAS评分均明显优于对照组,差异有统计学意义(t=2.849、2.021,P=0.005、0.043)。观察组并发症发生率(4.00%)明显低于对照组(18.00%),差异有统计学意义(χ^(2)=5.005,P=0.025)。结论在膝关节内侧间室骨性关节炎治疗过程中,全膝关节置换术可发挥确切疗效,可帮助患者更好地恢复膝关节功能。

Objective To explore the application value of total knee arthroplasty in the treatment of patients with osteoarthritis of the medial compartment of the knee.Methods A total of 100 patients with knee medial compartment osteoarthritis who were admitted to the hospital from October 2016 to November 2021 were included in the study.They were randomly divided into the control group and the observation group by double-blind method,with 50 patients in each group.Arthroscopic surgery and total knee replacement were performed.The treatment effects of the two groups were compared.Results The operation time,intraoperative blood loss,hospital stay and the first straight leg raising time in the observation group were significantly better than those in the control group,and the difference was statistically significant(t=9.563,6.020,4.126,6.726,P<0.001).After operation,the levels of C-reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the observation group were significantly lower than those in the control group,the difference was statistically significant(t=9.643,14.412,5.246,P<0.001).Compared with the control group,the WHOQOL-100 score of the observation group were significantly higher and PSQI score were lower,and the difference was statistically significant(t=4.649,8.060,P<0.001).The postoperative KSS score and VAS score of the observation group were significantly better than that of the control group,and the difference was statistically significant(t=2.849,2.021,P=0.005,0.043).The incidence of complications in the observation group(4.00%)was significantly lower than that in the control group(18.00%),and the difference was statistically significant(χ^(2)=5.005,P=0.025).Conclusion In the treatment of medial compartment osteoarthritis of knee joint,total knee arthroplasty can play a definite curative effect,and can help patients better restore knee joint function.
