胰岛素泵在糖尿病治疗中的临床疗效分析 被引量:2

摘要 目的分析糖尿病治疗中胰岛素泵的临床效果。方法选取2020年6月—2021年9月在该院实施治疗的糖尿病患者137例,以胰岛素用药方式不同予以分组,69例予以胰岛素泵治疗的患者归入治疗组,68例予以胰岛素皮下注射的患者归入对照组,对比治疗效果。结果两组患者治疗前血糖水平对比,差异无统计学意义(P>0.05);治疗后,与对照组睡前(8.25±2.35)mmol/L、餐后2 h血糖(8.25±1.65)mmol/L、空腹血糖(6.75±1.18)mmol/L比较,治疗组睡前血糖(6.55±1.35)mmol/L、餐后2 h血糖(7.18±0.95)mmol/L、空腹血糖(5.40±0.48)mmol/L均明显降低,差异有统计学意义(t=5.201、4.660、8.794,P<0.001)。治疗前血管内皮功能对比,差异无统计学意义(P>0.05);治疗后,与对照组NMD(17.10±1.65)%、FMD(5.05±0.68)%比较,治疗组NMD(21.10±2.25)%、FMD(7.35±0.75)%均明显较高,差异有统计学意义(t=11.852、18.796,P<0.001)。治疗组胰岛素平均用量为(32.18±1.35)U/d,对照组胰岛素平均用量为(43.35±1.45)U/d,差异有统计学意义(t=46.675,P<0.001)。治疗组血糖达标时间为(4.28±1.05)d,对照组血糖达标时间为(11.25±1.25)d,差异有统计学意义(t=35.359,P<0.001)。治疗组和对照组低血糖发生率分别为1.45%、14.49%,差异有统计学意义(χ^(2)=8.150,P=0.004)。结论在糖尿病临床治疗中予以胰岛素泵治疗效果突出,建议在临床治疗中推广。 Objective To analyze the clinical effect of insulin pump in the treatment of diabetes.Methods 137 diabetic patients who were treated in the hospital from June 2020 to September 2021 were analyzed.They were divided into different groups according to different ways of insulin administration.69 patients who received insulin pump therapy were included in the treatment group,and 68 patients who received insulin subcutaneous injection were included in the control group.Compared treatment effects.Results There was no statistically significant difference in blood glucose level between 2 groups before treatment(P>0.05);after treatment,compared with the control group at bedtime blood glucose(8.25±2.35)mmol/L,2 h after meal blood glucose(8.25±1.65)mmol/L and fasting blood glucose(6.75±1.18)mmol/L,Before sleep the levels of blood glucose(6.55±1.35)mmol/L(7.18±0.95)mmol/L 2 h after meal blood glucose and(5.40±0.48)mmol/L fasting blood glucose were significantly decreased,the differences were statistically significant(t=5.201,4.660,8.794,P<0.001).There was no statistically significant difference in vascular endothelial function before treatment(P>0.05);after treatment,compared with NMD(17.10±1.65)%and FMD(5.05±0.68)%of the control group,NMD(21.10±2.25)%and FMD(7.35±0.75)%of the treatment group were significantly higher,the differences were statistically significant(t=11.852,18.796,P<0.001).The average dosage of insulin in the treatment group was(32.18±1.35)U/d,and that in the control group was(43.35±1.45)U/d,the difference was statistically significant(t=46.675,P<0.001).The blood glucose standard time of the treatment group was(4.28±1.05)d,and that of the control group was(11.25±1.25)d,the difference was statistically significant(t=35.359,P<0.001).The incidence of hypoglycemia in the treatment group and the control group was 1.45%and 14.49%,respectively,the difference was statistically significant(χ^(2)=8.150,P=0.004).Conclusion The effect of insulin pump therapy in the clinical treatment of diabetes is outstanding,and it is recommended to be promoted in clinical treatment.
