摘要
目的 分析降钙素原与中性粒细胞百分比联合检测对糖尿病酮症酸中毒合并感染诊断价值。方法 回顾性分析该院2019年1月—2021年12月收治的78例糖尿病酮症酸中毒患者临床资料,根据患者院内感染发生情况分为参照组(未发生院内感染者,38例)与研究组(发生院内感染者,40例)。比较两组患者降钙素原检测水平、中性粒细胞百分比检测水平,分析降钙素原、中性粒细胞百分比联合检测的诊断价值。结果 研究组降钙素原、中性粒细胞百分比、C反应蛋白、降钙素原/乳酸水平明显高于参照组,差异有统计学意义(P<0.05)。降钙素原联合中性粒细胞百分比联合检测糖尿病酮症酸中毒合并感染阳性率高于降钙素原、中性粒细胞百分比单独检测,差异有统计学意义(P<0.05)。联合检验的曲线下面积(AUC)为0.624,灵敏度为77.50%,特异度为76.32%,约登指数78.69%。结论 糖尿病酮症酸中毒合并感染患者采用降钙素原与中性粒细胞百分比联合检测诊断灵敏度、特异度较高,对患者合并感染具有预警作用。
Objective To analyze the value of combined detection of procalcitonin and neutrophil percentage in the diagnosis of diabetic ketoacidosis complicated with infection.Methods The clinical data of 78 patients with diabetic ketoacidosis admitted to the hospital from January 2019 to December 2021 were retrospectively analyzed.According to the occurrence of nosocomial infection,the patients were divided into a reference group(patients without nosocomial infection,38 cases) and a research group(patients with nosocomial infection,40 cases).The detection levels of procalcitonin and the percentage of neutrophils were compared between the two groups.The diagnostic value of combined detection of procalcitonin and neutrophil percentage was analyzed.Results The levels of procalcitonin,neutrophil percentage,C-reactive protein,and procalcitonin/lactic acid in the reseach group were significantly higher than those in the reference group,and the difference was statistically significant(P <0.05).The positive rate of procalcitonin combined with neutrophil percentage combined detection of diabetic ketoacidosis complicated with infection was higher than that of procalcitonin and neutrophil percentage detection alone,and the difference was statistically significant(P<0.05).The area under the curve(AUC) was 0.624,the sensitivity was 77.50%,the specificity was 76.32%,and the Youden index was 78.69%.Conclusion The combined detection of procalcitonin and neutrophil percentage in patients with diabetic ketoacidosis complicated with infection has high diagnostic sensitivity and specificity,and has an early warning effect on patients with complicated infection.
