大承气汤治疗脊柱创伤后腹胀、便秘的临床疗效

Clinical Effect of Dachengqi Decoction on Abdominal Distension and Constipation after Spinal Trauma

目的 探究大承气汤治疗脊柱创伤后腹胀、便秘临床疗效。方法 选择该院2018年6月—2021年6月60例脊柱创伤后腹胀、便秘患者为研究对象,以随机数表法分为对照组、观察组,各30例,分别实施常规西医治疗、常规西医+大承气汤治疗,比较两组患者治疗效果、治疗前后中医证候积分及腰背部疼痛改善情况。结果观察组治疗有效率为96.67%,较对照组73.33%高,差异有统计学意义(χ^(2)=4.706,P<0.05);治疗前两组排便困难、大便性状、排便时间、排便不尽感、腹胀腹痛、肠鸣音积分相近,差异无统计学意义(P>0.05);治疗14、28 d时观察组排便困难度、大便性状、排便时间、排便不尽感、腹胀腹痛、肠鸣音积分水平均较对照组低,差异有统计学意义(P<0.05);治疗前两组腰背部疼痛视觉模拟法(VAS)评分相近,差异无统计学意义(P>0.05);治疗14、28 d时VAS评分水平较对照组低,差异有统计学意义(P<0.05);两组治疗期间均未见严重不良反应。结论 对脊柱创伤后腹胀、便秘患者常规西药治疗基础上,应用大承气汤治疗,可提升治疗有效率,降低中医证候积分,改善患者腰背部疼痛感受。

Objective To explore the clinical efficacy of Dachengqi Decoction in the treatment of abdominal distension and constipation after spinal trauma.Methods 60 patients with abdominal distention and constipation after spinal trauma from June 2018 to June 2021 in the hospital were selected as the research objects.Divided into control group and observation group by random number table method,30 cases in each group,received conventional western medicine treatment and conventional western medicine + Dachengqi Decoction treatment respectively.The treatment effect,TCM syndrome score and low back pain improvement before and after treatment were compared between the two groups.Results The effective rate of treatment in the observation group was 96.67%,which was higher than that in the control group,which was 73.33%,the difference was statistically significant(χ^(2)=4.706,P<0.05).Before treatment,the scores of defecation difficulty,stool character,defecation time,feeling of incomplete defecation,abdominal distention and abdominal pain,and bowel sounds were similar in the two groups,the difference was not statistically significant(P >0.05).14 d、28 d after treatment,the observation group had lower defecation difficulty,stool character,defecation time,feeling of incomplete defecation,abdominal distention and abdominal pain,and bowel sounds scores than those in the control group,the difference was statistically significant(P<0.05).Before treatment,the visual analogue scale(VAS) scores of low back pain in the two groups were similar,the difference was not statistically significant(P>0.05).The VAS score at 14 d and 28 d after treatment was lower than that in the control group,the difference was statistically significant(P<0.05).There were no serious adverse reactions in the two groups during treatment.Conclusion On the basis of conventional western medicine treatment for abdominal distention and constipation after spinal trauma,the application of Dachengqi Decoction can improve the treatment efficiency,reduce the TCM syndrome score,and improve the patient’s low back pain perception.
