无创呼吸机辅助治疗慢阻肺合并Ⅱ型呼吸衰竭的效果分析 被引量:4

Analysis of the Effect of Non-invasive Ventilator in the Treatment of Chronic Obstructive Pulmonary Disease Complicated with Type Ⅱ Respiratory Failure

摘要 目的 研究无创呼吸机辅助治疗慢阻肺(COPD)合并Ⅱ型呼吸衰竭的临床效果。方法 选取该院2018年6月—2021年5月接受治疗的78例COPD合并Ⅱ型呼吸衰竭患者为研究对象,采取随机数字表法分为两组,各39例。对照组采取常规治疗方案,观察组采取无创呼吸机辅助治疗。对比在不同治疗方案下两组患者的治疗有效率、生理指标、血气指标及肺功能。结果 观察组治疗总有效率(94.87%)高于对照组(79.49%),差异有统计学意义(χ^(2)=4.129,P<0.05)。治疗后,观察组生理指标(HR、RR)分别为(85.64±4.03)、(17.64±2.02)次/min,均低于对照组的(91.56±5.32)、(24.17±2.11)次/min,差异有统计学意义(t=5.539、13.961,P<0.05);观察组血气指标(SpO_(2)、PaO_(2))分别为(94.15±2.67)、(77.12±3.41)mmHg,均高于对照组的(88.16±3.56)、(70.56±3.05)mmHg;PaCO_(2)为(55.84±1.85)mmHg,低于对照组的(61.52±2.07)mmHg,差异有统计学意义(t=10.813、8.955、12.777,P<0.05);观察组肺功能指标(FEV1、FEV1/FVC)分别为(65.12±8.05)%、(65.78±5.81)%,均高于对照组的(56.22±7.31)%、(60.56±4.59)%,差异有统计学意义(t=5.111、4.403,P<0.05)。结论 无创呼吸机辅助治疗COPD并Ⅱ型呼吸衰竭效果显著,可改善患者生理指标及血气分析指标,提升患者肺功能。 Objective To study the clinical effect of non-invasive ventilator in the treatment of chronic obstructive pulmonary disease(COPD) complicated with type Ⅱ respiratory failure.Methods A total of 78 patients with COPD combined with type Ⅱ respiratory failure who were treated in the hospital from June 2018 to May 2021 were selected as the research objects.The patients were divided into two groups by random number table method,with 39 cases in each group.The control group was treated with routine treatment,and the observation group was treated with noninvasive ventilator.The therapeutic efficiency,physiological indexes,blood gas indexes and pulmonary function of the two groups of patients under different treatment plans were compared.Results The total effective rate of treatment of the observation group(94.87%) was higher than the control group(79.49%),and the difference was statistically significant(χ^(2)=4.129,P<0.05).After treatment,the physiological indexes(HR,RR) of the observation group were(85.64±4.03) times/min,(17.64±2.02) times/min,which were lower than those of the control group(91.56±5.32)times/min,(24.17±2.11) times/min,and and the difference was statistically significant(t=5.539,13.961,P<0.05).The blood gas indexes(SpO_(2),PaO_(2)) in the observation group were(94.15±2.67) mmHg,(77.12±3.41) mmHg,which were higher than those in the control group(88.16±3.56) mmHg,(70.56±3.05) mmHg;PaCO_(2) was(55.84±1.85) mmHg,lower than the control group(61.52 ±2.07) mmHg,the difference was statistically significant(t =10.813,8.955,12.777,P<0.05).The pulmonary function indexes(FEV1,FEV1/FVC) of the observation group were(65.12±8.05) %and(65.78±5.81)%,respectively,which were higher than those of the control group(56.22±7.31) % and(60.56±4.59)%,the difference was statistically significant(t=5.111,4.403,P<0.05).Conclusion Non-invasive ventilator adjuvant therapy for COPD complicated with type Ⅱ respiratory failure has a significant effect,can improve the physiological indexes and blood gas analysis indexes of patients,and improve the pulmonary function of patients.
