探讨早期应用猪肺泡表面活性物质治疗晚期早产儿呼吸窘迫综合征的临床效果

摘要 目的 评估早期应用猪肺泡表面活性物质治疗晚期早产儿呼吸窘迫综合征的临床效果。方法 选取2018年6月—2021年9月于该院行晚期早产儿呼吸窘迫综合征治疗的93例患儿。基于治疗方式差异分为对照组(46例)和观察组(47例)。对照组早期予常规治疗,治疗中期再予猪肺磷脂治疗;观察组就病症早期予猪肺磷脂治疗。比对两组治疗前后血气指标以及各项临床指标情况。结果 治疗后,观察组pH值(7.43±0.11)、动脉氧分压(79.02±2.12)mmHg及血氧饱和度(0.92±0.08)高于对照组(7.33±0.08)、(76.41±2.11)mmHg、(0.83±0.10),差异有统计学意义(t=-5.005、-5.950、-4.798,P<0.001);观察组症状缓解时间(4.11±0.38)d、机械通气时间(2.98±0.27)d及康复时间(9.13±0.87)d短于对照组(4.92±0.47)、(3.77±0.24)、(10.37±0.74)d,差异有统计学意义(t=9.149、14.902、7.396,P<0.001);观察组心率(126.53±7.65)次/min与呼吸频率(38.72±4.03)次/min低于对照组(132.27±7.31)、(45.39±4.78)次/min,差异有统计学意义(t=3.698、7.281,P<0.001)。结论 晚期早产儿呼吸窘迫综合征于治疗早期应用猪肺泡表面活性物质效果理想。 Objective To evaluate the clinical effect of early application of porcine alveolar surfactant in the treatment of late preterm infants with respiratory distress syndrome.Methods A total of 93 children with respiratory distress syndrome in late preterm infants who were treated in the hospital from June 2018 to September 2021 were selected as the subjects.Based on differences in treatment methods,they were divided into control group(46 cases)and observation group(47 cases).The control group received routine treatment in the early stage,and then received porcine lung phospholipid treatment in the middle stage of treatment.The observation group was treated with porcine lung phospholipid in the early stage of the disease.The blood gas indexes and various clinical indexes were compared between the two groups before and after treatment.Results After treatment,the pH value(7.43±0.11),arterial partial pressure of oxygen(79.02±2.12)mmHg and blood oxygen saturation(0.92±0.08) in the observation group were higher than those in the control group(7.33±0.08),(76.41±2.11)mmHg,(0.83±0.10),and the difference was statistically significant(t =-5.005,-5.950,-4.798,P <0.001);symptom relief time(4.11 ±0.38)d,mechanical vent ilation time(2.98±0.27)d and recovery time(9.13±0.87)d in the observation group were shorter than those in the control group(4.92±0.47)d,(3.77±0.24)d(10.37±0.74)d,and the difference was statistically significant(t=9.149,14.902,7.396,P<0.001);the heart rate(126.53±7.65)times/min and respiratory rate(38.72±4.03)times/min in the observation group were lower than those in the control group(132.27±7.31)times/min,(45.39±4.78)times/min,and the difference was statistically significant(t=3.698,7.281,P<0.001).Conclusion The early application of porcine alveolar surfactant is ideal in the treatment of late preterm infants with respiratory distress syndrome.
