脑出血合并糖尿病患者术后综合护理的应用及对生活质量的影响

Application of Postoperative Comprehensive Nursing in Patients with Cerebral Hemorrhage Complicated with Diabetes Mellitus and Its Influence on Quality of Life

目的研究术后综合护理在脑出血合并糖尿病中的价值。方法选择该院2018年3月—2021年2月纳入的76例脑出血合并糖尿病患者,按随机数表法分成两组各38例,研究组开展术后综合护理,对照组开展常规护理,干预前后测定两组空腹血糖、餐后2 h血糖,统计两组并发症(肺部感染、低血糖、压力性损伤)情况,干预前后进行汉密尔顿抑郁、焦虑(HAMD、HAMA)、生活质量(QOL)评分调查,护理结束后进行满意度评价,比较两组护理结果。结果研究组并发症发生率7.89%低于对照组(26.32%),但满意度(89.47%)高于对照组(68.42%),差异有统计学意义(P<0.05)。干预前两组血糖、HAMD、HAMA、QOL评分比较,差异无统计学意义(P>0.05);干预后研究组空腹血糖、餐后2 h血糖、HAMD、HAMA低于对照组,QOL评分高于对照组,差异有统计学意义(P<0.05)。结论术后综合护理的效果更为显著,能够控制血糖水平,并稳定患者情绪,缓解负性心态,提升生活质量,患者满意度高,且并发症少,值得推广。

Objective To study the value of postoperative comprehensive nursing in patients with cerebral hemorrhage complicated with diabetes mellitus.Methods A total of 76 patients with cerebral hemorrhage complicated with diabetes who were enrolled in the hospital from March 2018 to February 2021 were selected and divided into two groups with 38 cases in each group according to the random number table method.The study group was given comprehensive nursing care,and the control group was given routine care.Before and after the intervention,the fasting blood glucose and 2 h postprandial blood glucose of the two groups were measured.The complications(pulmonary infection,hypoglycemia,pressure ulcers)of the two groups were counted,and Hamilton depression,anxiety(HAMD,HAMA),and quality of life(QOL)scores were investigated before and after the intervention.Satisfaction evaluation was carried out after the nursing,and the nursing results of the two groups were compared.Results The occurrence rat of complications in the study group was 7.89%lower than that in the control group(26.32%),but the satisfaction rate was 89.47%,higher than that in the control group(68.42%),the difference were statistically significant(P<0.05).Before the intervention,there was no statistically significant difference in blood glucose,HAMD,HAMA,and QOL scores between the two groups(P>0.05).After intervention,fasting blood glucose,2 h postprandial blood glucose,HAMD and HAMA in the study group were lower than those in the control group,and QOL score was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion The effect of postoperative comprehensive nursing is more significant,which can control the blood glucose level.It can stabilize the patient's mood,relieve the negative attitude,improve the quality of life,high patient satisfaction,and fewer complications,which is worthy of promotion.
