瓜蒌薤白半夏汤加减治疗痰浊闭阻型胸痹的临床分析

Clinical Analysis of Modified Gualou Xiebai Banxia Decoction in the Treatment of Phlegm Turbidity Blocking Type of Chest Obstruction

目的探讨瓜蒌薤白半夏汤加减治疗痰浊闭阻型胸痹的临床疗效,以期为临床治疗痰浊闭阻型胸痹提供更多参考。方法选取该院2019年1月—2021年1月收治的90例痰浊闭阻型胸痹患者,按随机数表法分为对照组和观察组,各45例,对照组给予阿司匹林、阿托伐他汀、单硝酸异山梨酯等西医治疗,观察组在对照组治疗基础上采用瓜蒌薤白半夏汤加减治疗,两组患者均治疗1个月。比较两组患者治疗后的临床疗效率、治疗前后的身体困重、心悸、胸闷、胸痛等中医症状积分、心绞痛评分量表(GRACE)、西雅图心绞痛评分量表(SAQ)评分以及不良反应发生率。结果观察组临床总有效率93.33%高于对照组73.33%,差异有统计学意义(χ^(2)=6.480,P<0.05),与治疗前相比,治疗后两组患者身体困重、心悸、胸闷、胸痛评分、GRACE评分、SAQ评分均下降,观察组低于对照组,观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论瓜蒌薤白半夏汤加减治疗可有效缓解闭阻型胸痹临床症状,改善心绞痛,疗效确切且安全性较高。

Objective To explore the clinical efficacy of Gualou Xiebai Banxia Decoction in the treatment of phlegm turbidity blocking type of chest obstruction,in order to provide more references for the clinical treatment of phlegm turbidity blocking type of chest arthralgia.Methods 90 patients with phlegm turbidity blocking type of chest obstruction admitted to the hospital from January 2019 to January 2021 were divided into control group and observation group by random number table method,with 45 cases in each group.The control group was treated with aspirin,atorvastatin,isosorbide mononitrate and other Western medicine.The observation group was treated with modified Gualou Xiebai Banxia Decoction on the basis of the control group.Both groups were treated for 1 month.The clinical treatment efficiency,traditional Chinese medicine symptom scores such as body sleepiness,palpitation,chest tightness and chest pain before and after treatment,the scores of angina pectoris scale(GRACE),Seattle angina pectoris scale(SAQ)and the incidence of adverse reactions were compared between the two groups.Results The total clinical effective rate in the observation group 93.33%was higher than 73.33%of the control group,the difference was statistically significant(χ^(2)=6.480,P<0.05).The scores of body sleepiness,palpitation,chest tightness,chest pain,GRACE and SAQ in the two groups were decreased after treatment,the scores of the observation were lower than the scores of the control group,the incidence of adverse reactions in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Gualou Xiebai Banxia Decoction can effectively relieve the clinical symptoms of blocking type of chest obstruction and improve angina pectoris.It has good effect and high safety.
