达格列净联合益生菌对糖尿病性腹泻患者肠道微生态和胃肠功能及炎性因子的影响 被引量:6

Effects of dapagliflozin combined with probiotics on the intestinal microecology,gastrointestinal function,inflammatory factors in patients with diabetic diarrhea

摘要 目的探讨达格列净联合益生菌对糖尿病性腹泻患者肠道微生态、胃肠功能、炎性因子的影响。方法选取2017年4月至2019年4月糖尿病性腹泻患者138例,采用随机数字表法分为2组,每组69例。对照组采用阿卡波糖联合益生菌治疗,观察组采用达格列净联合益生菌治疗,于治疗前后行生化指标、肠道微生态、胃肠功能、炎性因子检测,比较2组治疗效果。结果2组治疗后空腹血糖、餐后2 h血糖、糖化血红蛋白、肠杆菌、肠球菌、胃动素、胃泌素、C-反应蛋白、肿瘤坏死因子α、白细胞介素-6低于本组治疗前,而双歧杆菌、乳酸杆菌、口-结肠运转时间、生长抑素高于本组治疗前(P<0.05)。观察组治疗后空腹血糖、餐后2 h血糖、糖化血红蛋白、肠杆菌、肠球菌、胃动素、胃泌素、C-反应蛋白、肿瘤坏死因子α、白细胞介素-6低于对照组(P<0.05),而双歧杆菌、乳酸杆菌、口-结肠运转时间、生长抑素高于对照组(P<0.05)。观察组总有效率高于对照组(P<0.05)。结论达格列净联合益生菌可改善糖尿病性腹泻患者肠道微生态,提高胃肠功能并减轻炎性反应,效果显著。 Objective To investigate the effects of dapagliflozin combined with probiotics on the intestinal microecology,gastrointestinal function,inflammatory factors in patients with diabetic diarrhea.Methods A total 138 patients with diabetic diarrhea who were treated from April 2017 to April 2019 were enrolled in the study,who were randomly divided into two groups,with 69 cases in eacg group.The patients in control group were treated by acarbose combined with probiotics,however,those in observation group were treated by dapagliflozin combined with probiotics.The biochemical indexes,intestinal microecology,gastrointestinal function,inflammatory factors,and curative effects were observed and compared between the two groups.Results After treatment,the levels of fasting blood glucose,postprandial 2h blood glucose,glycosylated hemoglobin,enterobacter,enterococcus,motilin,gastrin,C-reactive protein,tumor necrosis factorα(TNF-α),interleukin-6(IL-6)in both groups were significantly decreased,however,the levels of bifidobacterium,lactobacillus,oral colon transit time,somatostatin were significantly increased(P<0.05).Moreover the levels of fasting blood glucose,postprandial 2h blood glucose,glycosylated hemoglobin,enterobacter,enterococcus,motilin,gastrin,C-reactive protein,TNF-αand IL-6 in observation group were significantly lower than those in control group,while the levels of bifidobacterium,lactobacillus,oral colon transit time,somatostatin were significantly higher than those in control group(P<0.05).In addition the total effective rate in observation group was significantly higher than that in control group(P<0.05).Conclusion Dapagliflozin combined with probiotics can significantly improve the intestinal microecology in patients with diabetic diarrhea,which can also improve gastrointestinal function and relieve the inflammatory response of patients,with significant curative effects.
