喉癌术后患者疾病不确定感的影响因素

Influencing factors of uncertainty in illness in patients with laryngeal cancer after surgery

目的分析喉癌术后患者疾病不确定感的影响因素。方法选取2019年1月至2021年1月收治的206例喉癌患者作为研究对象,全部患者均行喉癌手术治疗,于出院前采用疾病不确定感量表(MUIS)评估喉癌患者术后疾病不确定感,并设计基线资料调查表,收集全部患者基线资料,比较不同资料特征喉癌患者术后MUIS评分,分析可能影响喉癌患者术后疾病不确定感的相关因素。结果206例喉癌患者术后疾病不确定感一般,MUIS评分平均得分(56.34±6.78)分;TNM分期为Ⅲ期、家庭月收入≤5000元、应对方式为消极应对、焦虑自评量表(SAS)评分≥50分、抑郁自评量表(SDS)评分≥53分的喉癌患者术后MUIS评分高于TNM分期为Ⅰ~Ⅱ期、家庭月收入>5000元、应对方式为积极应对、SAS评分<50分、SDS评分<53分的喉癌患者,差异有统计学意义(P<0.05);其他不同资料特征的喉癌患者术后MUIS评分比较差异无统计学意义(P>0.05);经多元线性回归分析结果显示,TNM分期为Ⅲ期、家庭月收入≤5000元、应对方式为消极应对、SAS评分≥50分、SDS评分≥53分可能是喉癌患者术后疾病不确定感一般的影响因素(P<0.05)。结论喉癌术后患者疾病不确定感水平一般,可能受TNM分期为Ⅲ期、家庭月收入≤5000元、消极应对、焦虑、抑郁等因素影响。

Objective To investigate the influencing factors of uncertainty in illness in patients with laryngeal cancer after surgery.Methods A total of 206 patients with laryngeal cancer who were treated by surgery in our hospital from January 2019 to January 2021 were enrolled in the study.Before discharging from hospital,Mishel uncertainty in illness scale(MUIS)was used to evaluate the uncertainty in illness,and the baseline data questionnaire was designed,the baseline data of patients were collected.After operation the MUIS scores of patients with different data characteristics were observed and compared to analyze the possible influencing factors of uncertainty in illness of patients.Results The uncertainty in illness in 206 patients after surgery was general,and the average scores of MUIS were(56.34±6.78)points.And the MUIS scores in patients with TNM stageⅢ,family monthly income≤5,000 yuan,coping style of negative coping,SAS score≥50 points and SDS score≥53 points were significantly higher than those of patients with TNM stageⅠ~Ⅱ,family monthly income>5,000 yuan,coping style of positive coping,SAS score<50 points and SDS score<53 points(P<0.05).There were no significant differences in MUIS scores of patients with different data characteristics after surgery(P>0.05).The multiple linear regression analysis showed that TNM stageⅢ,family monthly income≤5,000 yuan,coping style of negative coping,SAS score≥50 points,SDS score≥53 points might be the general influencing factors of uncertainty in illness of patients after operation(P<0.05).Conclusion The levels of uncertainty in illness in patients after laryngeal cancer surgery are general,which may be affected by TNM stageⅢ,family monthly income≤5,000 yuan,negative coping,anxiety,depression and the other factors.
