摘要
目的研究支气管镜技术在ICU重症肺炎患者中临床运用的影响。方法方便选取该院2019年3月—2021年3月期间ICU治疗的重症肺炎患者104例,通过随机综合平衡法分组,接受常规抗感染、机械通气治疗的52例患者为参照组,接受支气管镜技术治疗的52例患者为研究组,连续治疗半个月后评价组间临床疗效、炎症因子水平、治疗情况、肺通气功能、肺动脉血气指标。结果治疗后,研究组临床总有效率为98.08%,明显较参照组的86.62%高,差异有统计学意义(χ^(2)=4.378,P=0.014);研究组治疗后的TNF-α、hs-CRP、IL-6水平均显著较参照组低,差异有统计学意义(P<0.05);研究组治疗后FVC、FEV1、FEV1/FVC水平均显著高于参照组,差异有统计学意义(P<0.05);研究组治疗后的SaO_(2)、PaO_(2)水平均显著较参照组高,PaCO_(2)水平显著较对照组低,差异有统计学意义(P<0.05);研究组机械通气时间、肺部感染控制时间、ICU停留时间、治愈时间均明显短于参照组,差异有统计学意义(P<0.05)。结论在ICU重症肺炎患者的治疗中,支气管镜技术治疗可抑制炎症因子水平,有利于改善肺部通气功能与动脉血气指标。
Objective To study the clinical application of bronchoscopy in ICU patients with severe pneumonia.Methods Conveniently selected 104 patients with severe pneumonia who were treated in the ICU in the hospital from March 2019 to March 2021.By random comprehensive balance method,52 patients who received conventional anti-infection and mechanical ventilation therapy were included in the reference group.52 patients treated with bronchoscopy technology were included in the study group.After half a month of continuous treatment,the clinical efficacy,inflammatory factor levels,treatment conditions,pulmonary ventilation function,and pulmonary arterial blood gas indexes between the groups were evaluated.Results After treatment,the total clinical effective rate of the study group was 98.08%,which was significantly higher than the 86.62%of the reference group,the difference was statistically significant(χ^(2)=4.378,P=0.014).After treatment,the levels of TNF-α,hs-CRP and IL-6 in the study group were significantly lower than those in the reference group,the difference was statistically significant(P<0.05).After treatment,the levels of FVC,FEV1,FEV1/FVC in the study group were significantly higher than those in the reference group,the difference was statistically significant(P<0.05).The SaO_(2)and PaO_(2)levels of the study group after treatment were significantly higher than those of the reference group,and the PaCO_(2)level was significantly lower than that of the reference group,the difference was statistically significant(P<0.05).The study group's mechanical ventilation time,lung infection control time,ICU stay time,and healing time were significantly shorter than those of the reference group,the difference was statistically significant(P<0.05).Conclusion In the treatment of patients with severe pneumonia in the ICU,bronchoscopy technology can inhibit the level of inflammatory factors.It is beneficial to improve pulmonary ventilation function and arterial blood gas indicators.
